Caring for someone with dementia can feel overwhelming—but you don't have to navigate the journey alone.
The Medicare GUIDE (Guiding an Improved Dementia Experience) Program was created to support both individuals living with dementia and the caregivers who care for them. Medicine in Motion, an Omaha-based geriatric practice, has partnered with Comfort Keepers to bring these valuable services to families throughout Iowa and Nebraska.
Through the GUIDE Program, families are connected with a dedicated care navigator, often a nurse or social worker, who provides education, coordinates care, answers questions, and offers guidance every step of the way. The program also includes up to $2,500 annually in respite services through Comfort Keepers, allowing caregivers time to rest, recharge, and care for their own well-being.
For individuals experiencing memory changes who have Traditional Medicare, GUIDE is a no-cost program designed to provide personalized support, care coordination, and resources that help individuals remain safely and comfortably at home.
