A Release of Information form helps protect your privacy while allowing us to share or obtain your medical information when needed.

You may need an ROI form if you:

✅ Need records from an out-of-state healthcare facility
✅ Want copies of your medical records, lab results, vaccination records, or other health information
✅ Need records sent to or received from a Behavioral Health Clinic
✅ Want to authorize friends, family members, attorneys, or other individuals to access your medical records

Some record requests may have a service fee.
ROI forms are valid for one year unless you choose to revoke them sooner.

Due to current air quality conditions, Chairman Boyd, on behalf of the Executive Council, has announced that Tribal government buildings will be closed on Thursday, July 16, 2026.

‼️ Health & Wellness will remain open with limited staff to provide care for urgent needs only. ‼️

Please note:
• The Pharmacy will close at 5:00 PM.
• Non-urgent services may be delayed or rescheduled.
• The Recovery Resource Center will be closed.

If you are experiencing a mental health emergency please call or text 988.

Due to the poor air quality conditions, the Health and Wellness Center and the RRC will be closed for the remainder of the day on July 15th, 2026.

If you have an urgent need, please leave a message 715-478-4339, and a staff member will return your call as soon as possible. A limited number of employees will be monitoring messages periodically throughout the remainder of the day.

The Rabies Clinic scheduled for today will proceed as planned. The veterinarian who provides this annual service has traveled a significant distance to be here, and we appreciate their commitment to our community.
Due to the current poor air quality conditions, we encourage everyone attending to use caution while commuting to and from the Land & Natural Resources Building. Please take any necessary precautions to protect your health and safety during your travel.
