A Release of Information form helps protect your privacy while allowing us to share or obtain your medical information when needed.

You may need an ROI form if you:

βœ… Need records from an out-of-state healthcare facility
βœ… Want copies of your medical records, lab results, vaccination records, or other health information
βœ… Need records sent to or received from a Behavioral Health Clinic
βœ… Want to authorize friends, family members, attorneys, or other individuals to access your medical records

Due to current air quality conditions, Chairman Boyd, on behalf of the Executive Council, has announced that Tribal government buildings will be closed on Thursday, July 16, 2026.

‼️ Health & Wellness will remain open with limited staff to provide care for urgent needs only. ‼️

Please note:
β€’ The Pharmacy will close at 5:00 PM.
β€’ Non-urgent services may be delayed or rescheduled.
β€’ The Recovery Resource Center will be closed.
