Even thought the Geraldine Practice of Oakstone Family Health Clinic is closing Dr Ufford will continue to see his patients in all of the Long Term care Facilities and Assisted Living Facilities. He is going to focus his care in the Long Term Care Residents at Rehab Select Albertville Health & Rehab, Diversicare of Boaz, Marshall Manor Nursing and Rehabilitation Center, Crossville Health and Rehabilitation, LLC and Barfield Health Care. He will also continue care at the Assisted living facilities of Branchwater Village Independent & Assisted Living, Bridgewood Gardens and Arab Assisted Living. This will be his only focus after the clinic is closed, inpatient rehabilitation and long term and assisted living care. His compassion to care for the elderly is his greatest feature.

We are closing our practice Oakstone Family Health Clinic, in Geraldine, AL as of July 30th, 2026. We will no longer be able to serve as your Primary care physician after this date July 30th, 2026.

To ensure there is no interruption in your medical care, we strongly encourage you to establish a relationship with a new healthcare provider as soon as possible.

Your medical records will remain securely stored at Crossville Family Care in Crossville, AL. Dr Ufford, still collaborates with both Practitioners at Crossville Family Care. They are currently accepting new patients. If you would like your records transferred to them and start care in Crossville, we will be glad to help with a smooth transition of records and care.

If you would like your records transferred to your new physician, please contact us at 256-641-0520 and we will be happy to get your records faxed to your new provider. Your provider can also fax us at 256-661-8309 to obtain your records.
