🔦 Before you decide a before & after looks impressive, look at the photo itself.

Patients often scroll through a surgeon’s results trying to understand what may be possible for them. Board-certified plastic surgeon Dr. Joseph Kelling explains why standardized photography is so important when evaluating those images.

The goal is to make sure the change you’re seeing comes from the surgery, not from a different photography setup.

One of the biggest things to watch? Lighting. A darker before photo followed by a brighter, flash-lit after can reduce shadows and make wrinkles appear less noticeable before you’ve even evaluated the surgical result.

So when you’re comparing before-and-afters, look beyond the transformation. Pay attention to whether the lighting and color stay consistent from one image to the next.

The goal should be an objective comparison, not simply a more flattering photograph.

🔍 Upper and lower eyelid surgery treat the same general area, but the surgery and recovery can look very different.

With an upper blepharoplasty, board-certified plastic surgeon Dr. Joseph Kelling explains that treatment often involves removing a relatively small amount of excess skin, with a comparatively straightforward recovery.

The lower eyelid can be more complex. Under-eye bags, looseness of the lower lid, changes in eye shape and crepey skin may all be happening at the same time, which means the plan may need to address fat, support and skin quality differently.

Depending on what’s contributing to the concern, that can involve addressing the fat pads, tightening support around the lower eyelid, removing a small amount of skin, or incorporating a laser or chemical peel.

🔄 For a long time, Dr. Kelling wasn’t convinced the mini facelift had much of a role beyond marketing. But with more experience in facial rejuvenation surgery, he’s found that the right patient really can benefit from a more limited approach.

So what does a “mini facelift” actually mean in this context?

In the patients Dr. Kelling describes, it’s a more limited approach focused on earlier changes along the jawline and neck, while the mid-face structure and overall facial volume remain well maintained.

He’s seeing this most often in patients in their 40s, and occasionally late 30s, who still have a relatively youthful face but are beginning to notice looseness around the lower face, jawline, or neck.

For the right patient, “mini” doesn’t mean gimmicky. It means matching the extent of surgery to the extent of the concern.

🪄 Facial rejuvenation can involve more than one area, and this patient’s result is a great example.

This patient, in her late 60s, underwent a preservation deep plane face and neck lift with partial submandibular gland resection for lower facial rejuvenation.

To address the upper face as well, board-certified plastic surgeon Dr. Joseph Kelling also performed a minimal-incision brow lift with upper and lower blepharoplasty.

Rather than treating one feature in isolation, the surgical plan addressed the upper face, lower face and neck together for a more cohesive result.

A neck lift focuses below the jawline, addressing the neckline, heaviness, crepey skin and looseness of the platysma muscle that can create vertical neck bands.
