Most physicians treat the patient. Far fewer document the proof.
MEAT is the four-anchor framework CMS uses to decide if your HCC code pays:
Monitor - Is the condition being tracked over time?
Evaluate - What is the current clinical state?
Assess - What is the severity or trajectory?
Treat - What intervention is in place?
Miss even ONE of the four in a visit note, and the HCC code doesn't pay.
This isn't a billing technicality. It's the rule that converts clinical care into revenue.
