πŸ“‹ Right tissue. Right plan. Right surgeon. Dr. Rohrich closes out π™π˜½π™ƒ: π˜½π™§π™šπ™–π™¨π™© 𝙄𝙒π™₯𝙑𝙖𝙣𝙩𝙨 - Breast Reconstruction with his RJR Op Report, breaking down what really matters in implant-based reconstruction.

It starts with healthy, well-vascularized mastectomy flaps, because great reconstruction begins with great tissue. From there, the plan has to be individualized: above or below the muscle, ADM or synthetic scaffold, the right implant for that specific patient and anatomy. But the final piece is the one he calls most important of all, the surgeon. Experience, judgment, implant selection, and ex*****on are what ultimately determine the result. As he puts it: implants matter, technique matters more, but judgment matters most.

πŸ™ A huge thank you to our guest experts Dr. Maurice Nahabedian and Dr. Steven Sigalove, our scrub sink guest Dr. Essie Yates, and our hosts Dr. Bill Adams and Dr. Rod Rohrich for an incredible episode.

πŸ” Can you do a prepectoral reconstruction without ADM or P4HB? Dr. Adams brought that exact question to our expert panel on this episode of π™π˜½π™ƒ: π˜½π™§π™šπ™–π™¨π™© 𝙄𝙒π™₯𝙑𝙖𝙣𝙩𝙨 - Breast Reconstruction.

Dr. Nahabedian says yes, it's possible, and it may become more common as minimally invasive, robotic and endoscopic mastectomies grow, techniques where a small incision makes using ADM difficult in the first place. Early results are promising, with low rates of capsular contracture and malposition.

Dr. Rohrich raised a follow-up: if the mastectomy flaps are well vascularized, is a scaffold even necessary? Dr. Sigalove's answer: even well-perfused flaps benefit from what P4HB provides at a cellular level, upregulating anti-inflammatory and antimicrobial peptides that help guard against capsular contracture and infection. His take: skip the scaffold on poorly vascularized tissue, and the numbers will likely show it.

πŸ’¬ What do you think, would you want a scaffold used in your reconstruction? Let us know in the comments.

πŸ’— How far has breast reconstruction come in the last 30 years? πŸŽ—οΈ

On this episode of π™π˜½π™ƒ: π˜½π™§π™šπ™–π™¨π™© 𝙄𝙒π™₯𝙑𝙖𝙣𝙩𝙨 - Breast Reconstruction, Dr. Adams and Dr. Rohrich asked our guest experts to look back, and the answer is nothing short of remarkable.

Dr. Nahabedian has been doing this since 1992, and he'll tell you there is nothing he does today the way he first learned it. From implants placed completely under the muscle, to partial coverage, to ADMs, to helping pioneer the pre-pectoral movement over a decade ago, every shift moved toward better outcomes for the women in his care.

Dr. Sigalove put the bigger picture into words. Reconstruction used to mean giving a patient enough shape to look okay in a bra. Today, thanks to nipple-sparing mastectomies, better flaps, next-generation implants, and a full team working toward the same goal, patients are expecting, and getting, results that look virtually perfect. It's not just a better surgery. It's a better life on the other side of it.

🎬 This is the kind of progress worth seeing in full. Watch Episode 10 now, link in our bio and stories, or right here πŸ‘‰ https://youtu.be/j5vdUGUi6b4

What does ravioli have to do with breast implants? 🍝🫨

On this episode of π™π˜½π™ƒ: π˜½π™§π™šπ™–π™¨π™© 𝙄𝙒π™₯𝙑𝙖𝙣𝙩𝙨 - Breast Reconstruction, our panel breaks down where the name for this wrap technique actually came from.

Dr. Sigalove coined the technique back in 2015, a nod to medical school pathology days when everything got a food nickname. Think of the implant as the filling, and the mesh wrapped around it is the dough; hence, ravioli.

The reaction itself came from a red light green light breakdown by Dr. Andrew Cohen, showing a breast revision case using this exact wrap technique. Dr. Sigalove called the approach reasonable, noting that wrapping 360 degrees with ADM or P4HB has become common, and comes down to surgeon preference. Dr. Nahabedian gave it a green light too, calling downsizing a common, patient-driven operation, and ADM his go-to.

🧡 During Episode 10 of π™π˜½π™ƒ: π˜½π™§π™šπ™–π™¨π™© 𝙄𝙒π™₯𝙑𝙖𝙣𝙩𝙨 - Breast Reconstruction, Dr. Adams and Dr. Rohrich asked our expert panel a simple but important question: are they using P4HB exclusively now, or still alongside ADM?

Dr. Sigalove walked through his own shift. He used to wrap expanders in a hybrid approach, ADM on the front, P4HB on the back, and published results showing it worked well. Once lightweight P4HB became available, he dropped the ADM entirely. Since then, he's had zero flipping and says it's made his life way better.

"I love P4HB. I think it's one of the breakthrough products. It's been a total game changer," Dr. Nahabedian said. He leans on it most in complex revision cases, using it to stabilize an implant when ADM hasn't held up as well.

πŸ‘€ A viral claim from Terry Dubrow about implant complication rates made its way to our panel during π™π˜½π™ƒ: π˜½π™§π™šπ™–π™¨π™© 𝙄𝙒π™₯𝙑𝙖𝙣𝙩𝙨 - Episode 10: Breast Reconstruction - Individualized Outcomes, Informed Choices, and they had thoughts.

The claim: a 15 to 20% chance of complications with breast implants over time, plus a suggestion that switching to a specific implant is the fix.

Dr. Maurice Nahabedian agreed with part of the claim: implant surface can influence how much scar tissue forms. But he added an important nuance most people miss. Scar tissue isn't inherently bad. A certain amount of it is what keeps an implant stable.

Dr. Steven Sigalove pushed back on the numbers themselves, pointing out that a surgeon running a 20% complication rate wouldn't still be in business. His bigger point cut even deeper: no single implant is a shortcut to great results. Surgical technique is what actually determines the outcome, every time.

πŸ”¬ Implant technology doesn't stand still, and this episode of π™π˜½π™ƒ: π˜½π™§π™šπ™–π™¨π™© 𝙄𝙒π™₯𝙑𝙖𝙣𝙩𝙨 Episode 10 gets into what that progress actually looks like in practice.

🧼 In this scrub sink segment, Dr. Bill Adams asks the real question: have Gen 6 implants changed outcomes for reconstruction patients? Dr. Essie Yates breaks down why, pointing to more cohesive gel forms that hold upper pole fullness and cut down on secondary procedures. But Dr. Maurice Nahabedian flags the tradeoff, more projection can mean more flipping in pre-pectoral placements. Dr. Steven Sigalove has seen it too, but says newer cohesive options are finally striking the right balance.
