Here is an idea that really should not need saying. Good care should mean good care for everyone.

To every LGBTQ+ patient who has ever walked into a fertility appointment wondering if they would be truly seen. This season of ASRM Today is for you. Jeffrey Hayes and I have spent this season unpacking LGBTQ+ family building with the depth it actually deserves. The practical changes that make care feel safe instead of just survivable, sans barriers.

Because nobody should have to brace themselves walking into their own fertility appointment. You deserve providers who get this right. That is the whole point of this season.

I often tell patients that a semen analysis is a little like checking the engine before a road trip.
Before we decide where we're going, it helps to know how everything is running.

A semen analysis evaluates several key factors, including s***m count, motility (how s***m move), morphology (their shape), and volume. The test itself is simple, but the information it provides can be incredibly valuable when building a fertility treatment plan.

If you're trying to grow your family, this is often one of the most important first steps you can take. And once we have that information, we can sit down together, discuss your results, and explore the options that make the most sense for your path to parenthood.

Family building doesn't always follow the GPS.

For many LGBTQ+ individuals and couples, the route to parenthood may include a few extra turns, alternate roads, and unexpected stops along the way.

That doesn't make the destination any less meaningful.

PCOS (Polycystic Ovarian Syndrome) has had a bit of a branding problem for years.

The name made it sound like the condition was all about ovarian cysts when, in reality, it affects much more than that. Hormones. Metabolism. Fertility. Mental health. Even long-term health risks.

That's why experts from around the world have agreed on a new name (drum roll): PMOS (Polyendocrine Metabolic Ovarian Syndrome).

Think of it this way: calling it PCOS was a little like describing an entire house by only talking about the front door. Technically part of the story, but far from the whole picture.

The condition itself isn't changing, but the name better reflects what patients have been experiencing all along.

Families don't all look the same, and that's a beautiful thing.

I've learned that families are a lot like a great work of art. Some start with IVF. Some involve donor conception or surrogacy. Some take a path nobody could have predicted. Different colors, brushstrokes and different stories. Somehow, that's exactly what makes each one beautiful.

Here's to celebrating love, family, and the many ways both can grow. Happy Pride Month. 🌈

Eating well matters. The research on nutrition and fertility is real and I take it seriously.

But a life without joy in it is not actually a healthy life. Stress is inflammatory. Guilt is exhausting. And spiraling about Friday night pizza is genuinely worse for your body than the pizza.

So eat the tacos (or whatever your fun Friday food is) and enjoy every bite. Then come back Monday ready to go. Because moderation is not a compromise. It is what makes healthy living actually sustainable. That is called being human, which is actually the whole goal.

Now, if you need me I am being chased by a taco and it is gaining on me. Send help.

The physical journey gets the spotlight. The emotional one gets overlooked.

My wife and I are both medical providers. Between us we had every resource. Every contact. Every piece of clinical knowledge you could want walking into a fertility journey. And we still felt every single bit of the emotional weight. Because infertility does not care how many degrees are on your wall. The waiting is still the waiting. The grief is still the grief. The hope you have to keep choosing every morning is still a choice.

I became a better doctor the moment I became a patient. And part of what that means is that I never let the mental health piece get treated like a footnote.

Give your mental health the same care and intention you give your fertility treatment because one cannot thrive without the other.
