Chest pain can be scary.

It's always important to rule out more serious causes, sometimes the source is much more mechanical than people realize.

The costocartilage (or costochondral joint) is where the ribs connect to the sternum. When this area becomes irritated, it can create sharp, localized pain that often worsens with deep breathing, coughing, certain movements, or even pressing directly on the area.

I've seen it in runners, lifters, golfers, and even people who simply had a bad cough.

The good news is that once the source is identified, addressing the surrounding muscles, mobility, and movement patterns can often make a significant difference.

Most people have heard of TMJ issues.

But one of the most important muscles involved in chewing and jaw movement is actually the media (and lateral)l pterygoid.

This muscle sits deep inside the jaw and helps control opening, closing, and side-to-side movement. When it becomes tight or irritated, it can contribute to jaw pain, clicking, headaches, ear discomfort, and difficulty chewing.

Because of its location, it can often be overlooked during treatment.

One of the things I learned early in my Active Release Techniques training was how important this muscle can be for restoring normal TMJ function and reducing symptoms.

Running has been one of the greatest gifts in my life.

It took me from a kid running around Dallas to competing at Texas A&M and Colorado State. It taught me discipline through injuries, setbacks, heart surgery, and plenty of races that didn't go as planned.

Running introduced me to lifelong friends, helped shape my career, and eventually led me to become Chairman of the BMW Dallas Marathon...a race I dreamed of winning for more than 16 years before finally accomplishing that goal in 2015.

Today, I still love the simplicity of it. Put on a pair of shoes, step outside, and see where the road takes you.

Whether you're training for your first 5K, chasing a marathon PR, or simply running for your health, I hope you enjoy a few miles today.

Carpal tunnel syndrome involves irritation of the median nerve as it travels underneath the transverse carpal ligament at the wrist.

But symptoms like numbness, tingling, or weakness in the hand don’t always start there.

The median nerve travels from the neck through the shoulder, arm, and forearm before reaching the hand. Tightness or irritation anywhere along that pathway can contribute to symptoms.

Shoulder pain gets blamed on the rotator cuff all the time and sometimes that’s true.

But more often, it’s how the shoulder blade, upper back, and even the neck are working together that creates the problem.

If the shoulder blade isn’t moving well, the rotator cuff ends up doing more than it should. Over time, that’s when irritation shows up with reaching overhead, lifting, or even sleeping on that side.

Shoulder pain gets blamed on the rotator cuff all the time and sometimes that’s true.

But a lot of times, the bigger issue is how the shoulder, shoulder blade, neck, and upper back work together.

When one area gets tight or overloaded, something else usually has to compensate.

Treatment shouldn’t just focus on where it hurts...it should focus on why it started in the first place.

Not all “sciatica” starts in the low back.

Sometimes the issue is the piriformis a small muscle deep in the hip that sits very close to the sciatic nerve. When it gets tight or irritated, it can create pain, numbness, or tingling that travels down the leg and feels a lot like a disc issue. Sometime we use imaging to fully understand where the irritation is coming from but sometimes we can use tests and palpation to get a better understanding of where the source of the pain is coming from.

This is why treatment should always start with figuring out where the problem is actually coming from, not just where the pain shows up.

A lot of times, small changes in movement and the right hands-on treatment can make a big difference. Active Release Techniques has been really helpful in our treatment of specific types of pain similar to this.

A quick update for this Friday

I was scheduled to teach an ART seminar in Dallas this Friday, but the seminar didn’t end up needing an additional instructor....so we'll be back in the office Friday morning instead!
