The shiny lip is because of the skin glue I use to try to provide a little bit of extra support to the skin while healing.

I used to use removable sutures, but either we had to return to the OR to remove those sutures, or we'd hold the baby really snugly to remove them ever so gently in the clinic, but that was pretty traumatic for everybody - baby, parents, and surgeon.

The sutures along the rim of the nose are for the primary tip rhinoplasty, which seems to help both short-term and long-term.

This is offered to help provide realistic expectations that there is an in-between time, from surgery, to early recovery, through scar maturation and further growth, to reach our final result.

Dr. Law talks about his favorite instrument, the Penfield 1.

Not found in most Plastic surgery nor even Craniofacial sets, it comes from the neurosurgical world courtesy of Dr. Wilder Penfield.

One end hits the sweet spot of sharp but not too pointy, and the other end even more rounded for protecting delicate structures (like the nasal mucous or the brain) while dissecting.

It’s a gem of a tool worth trying out for that next alveolar bone graft!

On-table before and after of a cleft lip repair.

For incomplete clefts especially, there is a risk of creating a smaller nostril on the repaired side compared to the noncleft side.

During the repair, the dimensions of the noncleft side change, including the nostril and the lip.

If you try to match the new cleft-side nostril to the original noncleft side, it will end up too small because the noncleft side opens up to some degree as the columella pulls across towards the cleft side.

For this reason, I decide on the nostril sill resection - if any - after the lip muscle has been repaired.

This moment is also a time to double check the orientation of the alar insertion to be sure it’s not angled too high or too low.
