When should you request a Physiatry consult? 🩺

Sometimes the first sign that something has changed isn’t a new diagnosis—it’s a change in function.

New or worsening pain, decreased mobility, difficulty progressing in therapy, changes in transfers or ADLs, or an unexpected functional decline may signal that it’s time for a closer look.

Early Physiatry involvement can help identify barriers to recovery, address functional concerns, and support the resident’s ability to participate in therapy and daily activities.

Recovery isn't the result of one discipline working alone—it's the result of a team working together.

Therapists help residents build strength, improve mobility, and regain the skills needed for everyday life. Medical providers evaluate the clinical factors that may be limiting that progress, such as pain, changes in function, or other medical concerns.

When providers and therapists communicate regularly, treatment plans can be adjusted based on how a resident is progressing, helping remove barriers that may slow recovery.

That collaboration allows the entire care team to focus on the same goal: helping residents move more comfortably, participate more fully, and achieve the highest level of function possible.

At Paragon, we believe the best outcomes come from coordinated care—not isolated care.

A hospital discharge may close one chapter of care, but recovery is often just beginning.

When a resident enters skilled nursing, the care team must quickly understand what happened in the hospital, what has changed, and what could prevent the resident from safely moving forward.

That transition may include reviewing medications and diagnoses, identifying pain or mobility concerns, coordinating with therapy, setting functional goals, and monitoring the resident closely as needs evolve.

The strongest outcomes happen when nursing, therapy, and medical providers are aligned from the beginning.

The first few days after a hospital discharge are an important time for establishing a resident's plan of care.

As a resident transitions into skilled nursing, the interdisciplinary team begins gathering information, evaluating function, addressing medical concerns, and identifying anything that could affect recovery.

During those first several days, the focus often includes:

✔ Reviewing hospital documentation and medications

✔ Assessing mobility, pain, and functional status

✔ Beginning therapy and setting realistic goals

✔ Coordinating care among nursing, therapy, and medical providers

✔ Adjusting the plan of care as the resident's needs become clearer

Strong communication during this transition helps the care team respond to changing needs and support each resident's recovery journey.
