I’ve lived this from both sides. I’m a swallowing specialist, and I’m also a past NICU mom.

When you’re pumping around the clock, watching every ounce, and trying to maintain breastfeeding while also introducing bottles… the small details matter.

The ni**le you use can change the entire feed. I’ve seen it over and over again.

I like the Pigeon ni**les because they’re designed with intention around shape and softness.

The gradual slope and latch line help support lip placement and a deeper latch.

The silicone is soft and flexible, and for me, it’s allowed other family members to feed my baby while I’ve been able to continue breastfeeding without disruption. That became especially important when I went back to work and bottles became part of our routine.

My second baby is 5 months old and has done really well on the SS ni**le.

Ni**le selection isn’t based on age. It’s based on your baby’s skill set, their preference, and what allows them to feed well.

You can absolutely make bottles and breastfeeding work together in a way that feels good for you and your baby. 💕

The best outcomes happen when therapists, nurses, providers, and families work together, not in silos. Whether you’re partnering with families, defining your role, or navigating the “city rules” of your unit, remember: your voice matters as part of your team. So, speak up!

🩷Mentorship, open communication, and shared goals are what transform NICU care from overwhelming to empowering. When we lean on each other’s strengths, challenge outdated norms together, and support one another through the tough shifts, we create a culture where every baby, and every clinician, can thrive.

I know that may sound like a fairy tale to some of you... I've worked in that situation before too, but I'm living proof that it actually can become your reality with dedication, advocacy, and collaboration.

Not all breathing equals readiness.
Many NICU babies are "breathing on their own," but that doesn’t mean their airway, lungs, or neuroregulation are ready for oral feeding.

Here’s why...
🫁 Ventilation ≠ respiration.

Ventilation is moving air in and out of the lungs.

Respiration is gas exchange, the critical part that actually delivers oxygen to tissues and removes carbon dioxide.

You CAN have one without the other being fully functional.

And layering the demands of swallowing on top of an already taxed system is a recipe for respiratory distress.

A baby on CPAP or nasal cannula may appear stable, but still be struggling with gas exchange, airway protection, or regulation, especially if they've had trauma from intubation.

Translation? Even if the baby is “off the vent,” or "breathing fine," it doesn't mean they're ready to feed.

You’ve probably heard it before:
“She’s 37 weeks. She should be ready to feed.”
But here’s the truth we need to talk about👇

Feeding readiness isn’t just about gestational age.
It’s about development. Experience. Nervous system regulation.
And that timeline isn’t the same for every baby.

Some babies just need more time. Others need a different approach.

And all of them deserve care that meets them exactly where they are.
When we look beyond the clock and listen to the whole story... lungs, heart, brain, sensory history... we make safer, more compassionate decisions.

Feeding isn’t a checkbox. It’s a conversation, filled with compassion, centered around the family. A conversation you are qualified to lead.
