"Doc… be honest. Is PRP or stem cells better for my knee?"

I hear that question constantly — on the range, in the clubhouse, at random grocery store aisles.

Here's the truth: it's the wrong question.

PRP, MFAT (fat-based), and BMAC (bone marrow-based) are all legitimate tools. None of them is automatically the "starting point," and none of them is automatically the "upgrade" — no matter what the ads make it sound like.

In my latest video, I break down how I actually decide between the three:

A knee with decent joint space and mild to moderate wear often does well with a blood-based approach like PRP.

A knee that needs more biologic support, or where a prior treatment helped but didn't go far enough, may respond better to a fat-based option like MFAT.

A knee under heavier stress or more advanced damage may call for a bone marrow-based option like BMAC.

None of those is better or worse on its own. The right one depends entirely on the knee and the person attached to it.

IF YOU DO get a proper, golf-specific evaluation and match the treatment to what's really going on in your knee, you give yourself a real shot at delaying or even avoiding knee replacement.

IF YOU DON'T, you're just letting marketing pick your treatment while the joint quietly wears down.

Here’s what most golfers in that situation are never told: walking isn’t the problem. Stopping is.
Your knee cartilage has no blood supply. It feeds on movement. When you park yourself in a golf cart, you’re cutting off the food supply to the very tissue that’s supposed to cushion you.

Two conflicting messages are probably hitting your feed right now if you’re dealing with knee pain.
Message one: the traditional pipeline — pills, cortisone shots, then “let’s talk about surgery.”
Message two: ads for stem cell trips overseas, red light panels, and hyperbaric chambers promising a quick fix.
Here’s what 15+ years of treating golfers has taught me: the real engine of long-term knee improvement isn’t a bottled product shipped from someone else’s tissue. It’s built from YOUR OWN biology.
PRP uses your own platelets to calm inflammation. MFAT uses your own fat as a sticky scaffold to fill structural damage — like potholes in cartilage or a torn meniscus. Bone marrow concentrate goes even deeper when the bone itself is part of the pain.
Tools like shockwave therapy and medical-grade red light are excellent — I use them personally after my own treatments — but they’re boosters that support real biologic work, not replacements for it.
Before anyone brings up surgery, you deserve two things: proper imaging and an exam that maps exactly what’s causing your pain, and a real conversation about non-surgical options that use your own tissue.

Most golfers with mild to moderate knee arthritis have only been shown two doors:
cortisone now, surgery later.
Meanwhile, their feed is full of ads for PRP and every flavor of “stem cell” injection.
In my latest video, I break down how I actually answer that question:
• PRP, microfat (fat‑based stem cell–type treatments), and bone marrow are different tools, not a hierarchy.
• For many still‑active golfers with mild–moderate arthritis, PRP is often the smartest first move — your own blood, concentrated, with strong evidence behind it.
• Microfat and bone marrow are options I use for specific patterns of damage and specific goals, not because they sound fancier.
