A flexion-dominant baby comes in with a wide, flat head and a broad, low palate. The tongue cannot create enough suction to contact the palate effectively.

An extension-dominant baby looks totally different, narrow and elongated, with a high vaulted palate, and often presents with reflux, colic, and increased intracranial pressure.

When a baby is struggling to latch, the first recommendations are almost always about positioning. Try this hold. Try that angle. Use a ni**le shield. And sometimes those things help a little. But they never fix the root cause, because the root cause is structural.

The tongue, jaw, palate, and pharynx are all directly influenced by cranial and cervical subluxation patterns. When those patterns are present, feeding suffers, and no repositioning in the world will change that. This is what I teach in The Anatomy of Breastfeeding.
