Memorial Health is a not-for-profit organization that provides a full range of inpatient, outpatient, home health, hospice, behavioral health & primary care.

Our mission is to improve lives and build stronger communities through better health. Memorial Care on W. Hay is located at 441 West Hay in Decatur. The conveniently located office directly across from Decatur Memorial Hospital will offer in-person and virtual appointments. Schedule appointments on our app or at MyMemorialApp.com.

All Memorial Care and Memorial Specialty Care locations will be closed today due to weather and road conditions. If you need urgent care services, our South Sixth location in Springfield is open for walk-in. Drive-thru lab is closed.
