COMMUNITY PLANNING
NCA 310 develops a service plan, utilizing available resources, allowing people with intellectual and developmental disabilities the opportunity to live, work and play in th

eir community. PROGRAM SERVICES
The following services are available through NCA 310 and local providers:

Infant and Toddler
Infants and toddlers, age 0-36 months are provided case management and are referred to an Early Childhood Intervention program where the service coordinator, parent, and advisors implement a service plan to encourage the normal development of the child. ADULT SERVICES

Community Experience
People who are 21 or older are referred to a service provider for Community Experience. Training is provided to develop skills in all life activity areas. Employment
People are referred to a service provider for employment by their Support team. Basic work habits are taught and they are paid for the work they perform. Residential Services
Person's served by NCA 310 will be referred to a service provider providing residential services in a home setting if needed. They are encouraged to develop their maximum potential and enjoy being a citizen of their community. Service Coordination:
A NCA 310 Service Coordinator is provided to each person served. The Service Coordinator and support team decide what applicable services would best serve the person. They monitor these services to make sure their rights, as a person is not violated. INFORMATION AND REFERRAL
NCA 310 serves as the single point of
entry for the Alabama State Waiting List
for Morgan, Lawrence and Limestone Counties. For those families interested

Once a referral has been made with the Department of Mental Health Call Center the application is then forwarded to NCA 310 for processing. NCA contacts the family and requests the following information:

• A copy of a psychological evaluation and test results that occurred before the individual’s 18 birthday
• A copy of a psychological evaluation and test results after age 18
• Medicaid care and social security card
• If the individual is still in high school please provide copies of IEP. Once all pertinent records have been received, a Service Coordinator will meet with the family and complete additional required documentation. The Service Coordinator will process the referral to Regional Community Services where the person will be determined eligible or ineligible for the Alabama State Waiting List. Eligibility
Services are provided to people with intellectual and developmental disabilities living in Morgan, Lawrence and Limestone counties who meet the Department of Mental Health guidelines. Once a person is identified from the Alabama State Waiting List as eligible for services, a service coordinator is assigned to assist in finding the most appropriate services for the person being served by NCA 310.
