One of the more interesting areas in neuromodulation right now is not simply whether TMS works, but how treatment can be structured.

A 2026 randomized multisite study compared an accelerated H1-coil protocol with a standard TMS schedule. Both groups improved, while the accelerated group received multiple sessions per day rather than the traditional once-daily structure.

That does not mean “faster is automatically better.”

It means scheduling itself is becoming part of the clinical conversation.

For many patients, six weeks of weekday appointments is not a small logistical detail. Work, caregiving, transportation and distance all matter.

The useful question is not, “Which one is fastest?” It is, “Which clinically appropriate protocol can this person realistically complete?”

Psychiatry gets oversimplified when we reduce every medication trial to worked or failed.

A person may be sleeping better but still feel emotionally flat. They may have more energy but still struggle to enjoy anything. Anxiety may improve while depression remains.

That is a partial response, and it tells us something.

When we review treatment history, we are not only asking which medications were tried. We want to know what changed, what did not, what side effects appeared, how long the medication was taken, and what the person still wants to regain in daily life.

Sometimes the next step is another medication adjustment. Sometimes it is worth discussing a different treatment approach.

Good psychiatry starts with the details between “yes” and “no.”

Needing psychiatric support does not mean therapy has failed.

Therapy may be helping someone understand their patterns, improve relationships, respond differently to stress, or process painful experiences.

At the same time, persistent depression, anxiety, sleep disruption, mood changes, or medication concerns may be making it harder to use what they are learning in therapy.

The two forms of care can serve different purposes.

A psychiatric provider may help evaluate medication, clarify symptoms, or discuss interventional options while the therapist continues the deeper relational and emotional work already underway.

With the patient’s consent, those providers can work collaboratively while respecting the existing therapeutic relationship.

The goal is not to replace good care.

It is to make the care more complete and give the patient enough support to participate in it.

A patient can be interested in TMS and still need to know whether completing treatment is realistic.
That is not a lack of commitment.

Military families may be planning around duty schedules, civilian employment, childcare, transportation, distance from Decatur, upcoming travel, or a possible move.

They may also be trying to understand what TRICARE requires, who handles authorization, and whether an evaluation commits them to beginning treatment.

Those are not side questions. They are part of responsible treatment planning.
A plan can be clinically appropriate and still need to be adjusted around the patient’s actual life.

The best treatment conversation is not only, “Could this help?”

It is also, “Can we build a pathway you can realistically complete?”

Questions about SPRAVATO® are not signs that someone is resistant to care.

They are reasonable questions about a treatment that may feel unfamiliar.

Some patients worry about feeling out of control.
Some are afraid of dissociation or sedation.
Some are concerned about addiction because of what they have heard about ketamine.
Others are less afraid of the treatment than they are of the time, transportation, insurance, or what needing an advanced option might mean.

Those concerns deserve more than a reassuring phrase.

SPRAVATO® is administered in a certified healthcare setting, and patients are monitored for at least two hours. Transportation home is required, and patients should not drive until the following day after a restful night’s sleep.

“Let’s increase the dose.”
“Let’s give it more time.”
“Let’s try another medication.”

Sometimes one of those is the right next step. But persistent symptoms deserve a reassessment large enough to look at the whole picture.

What was tried?
Was the trial adequate?
Was there partial improvement?
Did side effects become the limiting factor?
Has the diagnosis or the patient’s life changed?
Is therapy helping, but untreated symptoms are still getting in the way?

Patients should not have to emotionally commit to a treatment before they understand whether they can realistically access it.

Insurance verification can help clarify:

Is the practice in network?
Is prior authorization required?
Will the service apply to a deductible or coinsurance?

What should the patient expect before treatment begins?

Medication management is not just deciding whether to increase, decrease, stop, or continue a prescription.

A useful appointment should make room for the questions that often get left behind.

What has actually changed?
Which symptoms remain?
What feels easier than it did three months ago?
What still feels hard to explain?
Are side effects affecting work, sleep, relationships, appetite, or motivation?
Does the treatment still support the life the patient is trying to live?

Sometimes the medication is helping, but not enough.
Sometimes the dose is stable while the person’s life is not.
Sometimes the most important thing a patient says comes after, “This might sound unrelated, but…”

Medication can be one helpful part of psychiatric care.

It should never replace listening, curiosity, or a broader understanding of the person taking it.
