Some moments remind us what person-centered care looks like in practice.๐
Recently, our Daytona team had the privilege of celebrating a remarkable milestone with our patient, Bernard, on his 100th birthday. ๐
Surrounded by his daughters and the caregivers who know him best, Bernard spent the day sharing stories, laughter, and the quick wit that continues to brighten every room he enters.
One of his favorite surprises was a Snoopy birthday cake, chosen especially for him because Snoopy has long been one of his favorite characters.
For healthcare professionals, moments like these serve as an important reminder that quality care extends beyond managing symptoms.
Hospice teams work to understand each patient's goals, preferences and sources of joy, helping support quality of life while providing comfort-focused care.
It is one example of how hospice care can support connection, dignity and moments that matter throughout a patient's journey.
Happy 100th Birthday, Bernard.๐
