CMS has released the FY 2025 Hospice PEPPER reports, and they are now available for download through the PEPPER portal.

What Is PEPPER?
The Program for Evaluating Payment Patterns Electronic Report (PEPPER) is a free comparative data report that summarizes hospice-specific Medicare claims data in target areas identified by CMS as potentially at risk for improper payments.
PEPPER is designed to help hospice leaders identify billing and utilization patterns that differ from state, Medicare Administrative Contractor (MAC) jurisdiction, and national benchmarks. The report serves as a valuable compliance and operational tool, helping organizations prioritize auditing, monitoring, and quality improvement efforts.

Why Hospice Leaders Should Review It Now
PEPPER provides three years of historical data, allowing organizations to evaluate trends over time and identify areas that may warrant closer review.
The report can help hospices:
• Monitor billing and utilization patterns
• Identify changes in length-of-stay trends
• Evaluate performance against state, MAC jurisdiction, and national benchmarks
• Prioritize internal auditing and monitoring activities
• Identify potential coding, documentation, or billing vulnerabilities
• Demonstrate proactive compliance oversight

As regulators continue to rely on data analytics to identify potential fraud, waste, and abuse, the same patterns highlighted in PEPPER are often the focus of external reviews and audits. Reviewing your report, understanding the underlying drivers, and addressing any unusual findings can help reduce organizational risk and strengthen compliance efforts.

MAC Legacy is a WBENC-certified Women’s Business Enterprise and a minority-owned company. That’s not something we lead with everyday, but it does matter in ways that aren’t always obvious.

Across healthcare, there’s been a steady shift toward working with certified partners. Many organizations through state programs, health systems, and grant-funded initiatives, are encouraged (and sometimes financially supported) to partner with women- and minority-owned businesses. There are a lot of programs tied to supplier diversity goals, community impact, and grant funding that make those partnerships meaningful.

But for us, this isn’t about checking a box. It’s about the kind of company we’ve built and the experience behind it.

MAC Legacy was built out of real provider operations. Our leadership team has spent decades in home health, hospice, and home care, not as observers, but as operators running multi-site agencies, navigating audits, managing clinical teams, and working through the same challenges our clients face every day.

That background is what shapes our work now.

We support organizations through:
- Coding and OASIS review
- Plan of Care and documentation support
- ADR/medical review and appeals
- Audits, mock surveys, and compliance support
- Operational and leadership consulting

It’s practical, hands-on work, done by people who understand both the clinical side and the financial impact.

The certification may open a door, but it’s the depth of experience and how we show up that really matters.

If you ever need documentation for vendor requirements or you’re exploring grant opportunities that prioritize certified partners, we’re happy to help. And if not, that’s okay too, we’re still here doing the work the same way we always have.

F&TS: same conference you love… just a new name

Alright friends, we need to talk about F&TS.
Yes, they’ve rebranded. Yes, it took us a second too, but it’s still the same conference you know.
We’re talking about the Finance & Technology Summit, formerly the Financial Managers Conference (FMC). Same great content, same smart people, just a name that better fits where the industry is headed.
And if you’ve been in this space for a while, you already know, this has been the go-to event for home health and hospice financial leaders for over 20 years.
Our founder, Marcylle Combs, has been coming to this conference in just about every role you can imagine; provider, owner, consultant, and now as an HHFMA Advisory Board member. She’s seen everything from paper billing to PDGM to where we are today with AI and automation.
👉 Hit play to hear why this is one event she doesn’t skip.

MAC Legacy at F&TS 2026
July 12–14, 2026 | Boston Marriott Copley Place
We’re not just attending, we’re showing up ready to share what’s actually working:

Marcylle Combs & Valerie Cornett
“Maximizing Employee Retention Using Data”
Tuesday, July 14 | 11:30 AM – 12:30 PM EDT
They’ll walk through how to use real data, burnout indicators, overtime, documentation lag, PTO, productivity, to spot turnover risks early (before they become expensive problems).

Lisa McClammy — Home Health Summer Camp (Preconference)
Sunday, July 12 | 9:00 AM – 3:00 PM
A deep dive into operations, reimbursement strategy, workforce, and care delivery.

If you’re trying to figure out how to keep your agency strong and sustainable right now, this is the room to be in.
We’ll be there.
Hope to see you there too.

We’re excited to announce that Chris Gallarneau is stepping into the role of Senior Director of Partnerships at MAC Legacy.

This move reflects both our continued growth, and the impact Chris has already made. Chris brings strong industry experience from his time in provider roles within hospice, along with a long-standing connection to our team, having previously worked alongside Marcylle during our FMS days.

He understands our work, our clients, and the expectations we hold and he’s proven that through the way he shows up and delivers. As we continue to grow, it made sense to elevate someone who already knows our standards and can help strengthen client relationships while identifying opportunities to expand how we serve.

Congratulations, Chris, we are excited to have you in this new role and a valued member of our team!

Leadership Lesson with Marcylle: Truth Builds Teams
In our industry, we say we want the truth, whether it’s in a chart, an audit, a survey, or a hard conversation but the truth isn’t always easy to hear, and it’s even harder to handle well. I’ve seen it over the years in home health and hospice, feedback comes in, a denial hits, documentation doesn’t hold up, or a gap is identified and our first response often isn’t ownership, it’s reaction. But maturity, real maturity, isn’t proven when things are going right; it shows up in how we respond when we’re challenged.

Truth, when it’s given in love and respect, is one of the most important tools we have. It protects our patients, strengthens our compliance, sharpens our clinical judgment, and ultimately leads to better outcomes. But receiving truth requires humility. We can choose to defend, justify, or deflect or we can pause, listen, and take responsibility for our part. That’s where motivation really takes hold, not from praise alone, but from being stretched, corrected, and made better.

If we want to build a stronger culture, we have to get better on both sides of this. We have to be willing to speak truth clearly and respectfully and we have to be just as willing to receive it without shutting down. That doesn’t happen by accident. It takes intention in how we lead, how we communicate, and how we show up for one another every day.

So this week, take a moment and ask yourself:
When I receive feedback on my work, what is my first response?
Am I truly listening, or am I preparing my defense?
Is there a conversation I’ve been avoiding because it feels uncomfortable?
Am I delivering truth in a way that reflects respect and a desire to help someone grow?
Where could stronger ownership on my part improve outcomes for my team or our clients?

Call to action: This week, have one real conversation you’ve been putting off, handle it with clarity, respect, and purpose. And when feedback comes your way, resist the urge to react, pause long enough to consider what you can take ownership of. That’s where growth starts.
