Located on the Eastern Shore of Maryland, Choptank Community Health System, Inc. (CCHS) is a private, non-profit community health center providing primary health care services in Caroline, Dorchester, and Talbot Counties and the surrounding areas. CCHS health centers provide a full range of primary medical, dental, and behavioral health services. CCHS is a participating provider with most major insurance companies and HMOs. In addition, a sliding fee scale is available for any patient without insurance. As a community health center, CCHS is able to use federal funding to expand current services and develop new programs to meet the health care needs in our area. Choptank Health encourages families, youth, and advocates to join Mid Shore Behavioral Health’s Hub Advisory Board meeting to share experiences and ideas for improving school and community mental health services.

Healthy habits can help the whole family feel better during the holiday season. Choptank Community Health System encourages families in Caroline, Kent, Queen Anne’s, Talbot, and Dorchester counties to make time for sleep, handwashing, movement, balanced meals, and conversations about feelings.

For younger children, steady routines offer comfort during busy days. For teens, time to recharge and breaks from screens can help manage seasonal stress.

Need support? Choptank Health welcomes new patients at all locations. School-based health services also help students get care while coordinating with their primary care provider. Families can enroll students throughout the year.

More care, closer to students. Choptank Community Health System is expanding school-based services with two new providers!

Kaitlin Connor, LMSW, provides behavioral health care at Sudlersville Elementary and Middle schools. Caitlin Hohenstein, RDH, joins our school-based dental program serving students in Kent and Queen Anne’s counties.

Both help students get care where they learn, making access easier for families.
