HEALTHeLINK serves as a resource for broad health system transformation through secure data-informed decision making for individual patient care and to support community investments with a purpose of improving health, health care, and collaboration with local health care providers and those who are connected via the Statewide Health Information Network for New York (SHIN-NY) to improve health outcomes and health care delivery, control costs, increase access to care, and improve health equity in Western New York. HEALTHeLINK is also a founding member of Civitas Networks for Health, a national collaborative working to use health information exchange to improve health.

Having these conversations with your family and making end-of-life decisions are important. Now providers can more easily access these critical documents and ensure patients’ wishes are known at the point of care, especially when they may be unable to speak for themselves.

Advanced care planning documents can’t be followed if they can’t be found. HEALTHeLINK is tackling this nationwide problem with a local solution.

We’re the only organization to receive a two-year federal grant to improve digital access to advance care directives.

By integrating advance care planning functionality directly into our platform, we’re taking the guesswork out of critical care moments.

Now, Western New Yorkers’ end-of-life care documentation is more easily available when it matters most, ensuring patients’ voices are always heard, even when they can't speak for themselves.

We’re making great progress loading historical data from multiple data sources into the new Fusion platform. As each source completes its loading, the data is available and participants will receive both an email and portal message when each source is ready to use.

HEALTHeLINK is putting the data to work for the WNY Integrated Care Collaborative. As part of a collaborative project, we are working with WNYICC to utilize HIE data to evaluate early results of the social care network and the impact it has had on regional health improvement, specifically with Medicaid members.

In running the region’s social care network, WNYICC and its partners have so far screened more than 65,000 Medicaid members across Erie, Niagara, Cattaraugus and Chautauqua counties, said Nikki Kmicinski, CEO of WNYICC.
