Dear Patient:
It is with mixed emotions that I share with you that I will be retiring effective January 31, 2023. While I am looking forward to spending more time with my husband, our dog Mollie, our friends and family, traveling, and pursuing my hobbies, I am deeply saddened that I will no longer be providing your medical care.
Before my last day seeing patients, Michelle (my Office Manager) and I will assist you with locating a new physician, transferring your care, and any billing questions you may have. Attached is a list of local physicians who may be able to meet your continued medical care needs. Please be aware that if you transfer to an Advocare doctor you will not need to have your medical records transferred as he or she will have access to the same electronic health record system which I use and, therefore, your complete medical record.
