Hospice care often begins with a simple phone call. A referral can come from a medical professional, community member, friend, family member, or even the patient. Anyone can take that first step. Once a request is made, CCS Hospice will connect with the patient, family, and physician to determine if hospice care is the right next step. We are here to support you every step of the way.

We honor the strength of survivors, support those currently facing breast cancer, and remember the loved ones we’ve lost. Early detection saves lives; regular screenings and awareness make all the difference.
