That’s why squats, creams, and weight loss don’t fix it and why so many patients feel like they’ve failed at something that was never solvable that way.

Those dimples are tethers. Fibrous bands called septae anchor the skin down to deeper tissue. Fat pushes up between them. The skin pulls in. You get a dimple.

Think of a cushion on a worn-out couch frame. The cushion looks flat, but the cushion was never the problem.

Under your lips sits a layer of support tissue (the SMAS) and deep pockets of fat. Both thin out with age. Most treatments only chase what you can see on the surface, so the result never quite looks right.
