Location: Duluth, MN / Superior, WI

Home Health Aide Job Description:

Pathways to Achievement Home Health has an opening for a part-time Home Health Aide. The HHA will provide visits for personal care services to patients in their home during daytime hours Monday- Friday. We are a Medicare Certified Home Health Agency, licensed in Minnesota and Wisconsin. Applicants with a variety of work experiences are encouraged to apply.

Qualifications/Requirements:
• Good interpersonal and communication skills
• Ability to work both independently and cooperatively in a
team setting
• Possess basic computer skills
• CNA certification from an accredited program is required
(Wisconsin certification is a plus)

Job Status: Part-time

Schedule: Daytime hours, Mon-Friday

Essential Job Duties/Functions:

1. Provides services and education/training to clients in their homes following the Physician's Plan of Care and the specific functions identified in the therapy care plan.
a. Evaluates the effectiveness of the current plan and client progress.
b. Completes visits in a timely manner and in accordance with agency policy.
c. Documents all services provided, observations, client progress, and recommendations according to established policy. Submits documentation to the agency per guidelines.
d. Reports changes to the therapist and/or the nursing supervisor in a timely manner.

2. Participates in interdisciplinary activities per agency guidelines.
a. Attends client care conferences as needed to provide information to other members of the team.
b. Promotes referrals to other disciplines as appropriate.
c. Attends staff meetings and continuing education programs.
d. Participates in the agency quality improvement activities pertinent to the role of Physical Therapy-Assistant.

3. Promotes personal safety and a safe environment for clients and coworkers.
a. Demonstrates knowledge of safety and infection control practices by compliance with agency policies and procedures.
b. Recognizes and reports potentially unsafe situations.
c. Demonstrates safe practice in the use of equipment.
d. Assesses the environment and initiates actions to prevent accidents and promote safety.

4. Maintains confidentiality in all aspects of the job.
a. Respects the confidentiality of information in the client's clinical record and shares this information in accordance with agency policy.
b. Does not discuss client or staff identities, conditions, or circumstances in public or with others who are not professionally involved with the situation/person.
c. Protects written confidential documents in a manner that prevents unauthorized access.

5. Performs the job in compliance with agency policies and procedures and professional and community standards.
a. Accepts responsibility in accordance with the role of Physical Therapy-Assistant
b. Provides services in accordance with applicable state, federal, and JCAHO standards.
c. Attends meetings and required educational programs as required.
d. Accepts responsibility for personal and professional development. Identifies learning/developmental needs.

6. Performs additional duties and responsibilities as deemed necessary.

Qualifications:
1. Licensed to practice as a Physical Therapy-Assistant in the state(s) of employment.
2. Graduate of a two-year college level program approved by the American Physical Therapy Association.
3. Minimum of six (6) months experience as a Physical Therapy-Assistant.
4. Good written and verbal communication skills and interpersonal skills.
5. Current driver’s license, good driving record, and reliable transportation.

Job Summary:
Plans and provides physical therapy services to clients in their homes in accordance with the Plan of Care and agency policies and procedures. The Physical Therapist performs functions that require substantial specialized knowledge, judgment, and skill based upon the principles of psychological, biological and social services and must be able to make judgments accordingly.

Reports to: Director/Clinical Supervisor/Case Manager

Qualifications:
1. Graduate of a Physical Therapy program approved by the American Physical Therapy Association, the Committee on Allied Health Education and Accreditation of the American Medical Association, or the Council on Medical Education of the American Medical Association and the American Physical Therapy Association.
2. Current license/registration as a Physical Therapist in Minnesota. Must be eligible to obtain Wisconsin licensure.
3. Minimum of one (1) years’ experience in a health care facility. Home care experience preferred.
4. CPR certified.
5. Strong verbal and written communication skills and good interpersonal skills.
6. Knowledgeable in the principles and practices of physical therapy.
7. Current driver’s license, good driving record, and reliable transportation.
8. Basic Computer Proficiency required
