Session: Competence Requirements for Managing Low-Acuity EMS Calls: Developing a Competence Framework for Community and Prehospital Care

Session: A Community Paramedicine Transitional Care Program to Support Early Emergency Department Discharge

The hours and days after an early emergency department discharge can determine whether recovery continues at home or the patient returns to crisis.

At IRCP 2026, Judah Goldstein, P*P, PhD, and Sean Collins, ACP, BScP, will present a community paramedicine transitional care programme launched in Halifax in 2023.

The program supported 108 patients, many older and moderately frail, through reassessment, coaching, laboratory services, and coordinated follow-up. Fourteen percent returned to the emergency department within one week, offering important lessons for program design and scale.

Frequent EMS use is rarely a simple matter of call volume. It often signals vulnerability, unmet need, fragmented care, and a system asking the wrong questions too late.

At IRCP 2026, Prof. Patrick Ristau, PhD., M.A., M.Sc., will examine preventive care needs among frequent EMS users, drawing upon findings from a rural Bavarian EMS district.
The session will explore epidemiology, case management, and the potential role of community paramedicine in creating more appropriate, coordinated responses for patients whose needs extend beyond a single emergency encounter.
