Session timing is a clinical variable in Parkinson's Disease and it is frequently set by scheduling availability rather than by clinical reasoning. Motor fluctuation means function varies substantially across the day. A patient seen during a reliable on period is observed and treated at their best, which produces accurate documentation of that window and no information about the state in which falls actually occur. Medication management belongs to the physician. Scheduling therapy relative to medication effect, and training in the environment and at the hour when movement is hardest, sits squarely within rehabilitation scope. Neuro Nation provides in-home neurologic physical and occupational therapy across the Triangle, NC and Jacksonville, FL. No physician referral is required to begin an evaluation.

Shared scheduling is not ok. When an outpatient therapist is assigned two or three people in the same hour, the session that results is supervised exercise rather than skilled intervention. This is a staffing model, not a clinician failure. Motor relearning after stroke depends on a trained observer watching each repetition and progressing the next one, and divided attention makes that grading impossible. The second issue is insurance authorization. When an episode of care ends because a plan stopped approving visits, an administrative schedule has effectively made a clinical decision about a person's recovery. Families are entitled to see both of those clearly. See how Neuro Nation works differently. We provide in-home neurologic physical and occupational therapy across the Triangle, NC and Jacksonville, FL. No physician referral is required to begin an evaluation.

From the outside a stiff arm looks like a stiff arm, but rigidity and spasticity are different mechanisms with different treatments. Spasticity is speed dependent: move the limb quickly and it catches, move it slowly and it gives. Rigidity resists at every speed, the same the whole way through, and that is the Parkinson's Disease pattern. The distinction changes the plan, because treating rigidity like spasticity produces a stretching program that does not change how he moves. Rigidity responds to large-amplitude movement and rotation, not to stretch alone. Ask which one it is. You notice it first, every time. The slower morning, the hand that stopped swinging, the second attempt at the chair, often weeks before any of it reaches an appointment. A clinic visit is a snapshot, usually taken during a good hour, so change that accumulates across weeks is invisible in that window. Write it down with a date. Three lines is enough: what changed, when you first noticed it, and how often it happens now. A date turns a worry into data, because "he seems worse" is hard to act on and "he has needed two tries at the chair since the fourth" is a clinical finding. Your notes travel further than the visit does. In-home neurologic PT and OT for Parkinson's Disease across the Triangle, NC and Jacksonville, FL.

You want to help your dad between sessions, so you are doing the sheet on the fridge. Ten reps, three times a day. Here is what nobody told you. That sheet was written to be safe for somebody to do alone, without a therapist watching. Easy enough that it cannot go wrong, which also means easy enough that it mostly is not changing anything. His body adapts to hard, and the sheet is not hard. What you want is somebody showing you, how to make it harder safely, and how to tell the difference between good hard and bad hard. Eleven months after his stroke he was living on one floor. The bedroom was upstairs and nobody had been up there with him since March. Stairs had been ruled out early, which was reasonable at the time and never revisited once he got stronger. So we trained on the stairs: two months, three days a week, his staircase with the railing, one tread at a time and with his daughter learning the guard. He sleeps upstairs now. Fourteen steps, one rail, no assist. Not every story goes like this and we will never promise you one that does. What we will do is test the ceiling rather than assume it. You want your dad walking like himself again, not shuffling. So you remind him. Pick up your feet, stand up straight, swing your arms. And he does, for about four steps. Here is why that doesn't work. Parkinson's Disease shrinks movement without telling him it is shrinking. His small steps feel normal sized from the inside, so a reminder only works while he is thinking about it. What changes it is training too big on purpose. Exaggerated arm swing, exaggerated step, over and over, until big recalibrates to normal. In-home neurologic PT and OT for Parkinson's Disease across the Triangle, NC and Jacksonville, FL.

Families often think choosing a new provider means firing the one they have, and that belief keeps people from asking for more. Care can be added. A second set of hands in the home does not cancel an outpatient plan, a home health episode, or a neurologist. The gap is usually dose rather than competence: the first team is doing good work at the frequency their setting allows. We fill the gap in care, 2-3 days a week, 60 to 90 minutes, one therapist and one patient the whole session. Keep the care you have and add the dose we provide. Intensive in-home neurologic PT and OT across the Triangle, NC and Jacksonville, FL. No physician referral is required to begin an evaluation.

In atypical parkinsonian syndromes, limited medication response is frequently communicated to families as there is nothing to be done. That idea is wrong. Poor medication response and faster progression are accurate statements about the disease(s) and its medication management. They are not statements about function, safety, or quality of life, which are the targets of skilled physical and occupational therapy. Protecting available movement, matching equipment to current status, modifying the home to reduce fall consequences, and training caregivers in safe transfer technique are all achievable regardless of medication response. Neuro Nation provides in-home neurologic physical and occupational therapy. No physician referral is required to begin an evaluation.

Your mom can walk beautifully down a therapy hallway and still not get to her own bathroom at night. That is not her failing. That is where the work happened. A hallway is flat, wide, lit, and clear. Her house has a rug, a doorway, a dog, and a turn she makes in the dark. Those are different skills, and the brain treats them as different skills. So when somebody says she is doing great in therapy and you are watching her struggle at home, you are both right. The gains are real. They got built somewhere she does not live.
