After months of planning, conversations, and bringing together an incredible team of professionals, we're officially launching ACAP SECT–River Valley.

This isn't just another program.

Beginning this week, families throughout Middlesex and New London Counties will have access to trusted education, local experts, practical resources, and a community that understands the journey.

Here's to helping families find answers a little sooner, feel a little less alone, and navigate aging with greater confidence.

Sometimes the right resource arrives exactly when a family needs it most. I hope ACAP becomes that resource for our community.

Last night was everything a Purple Party should be—filled with laughter, friendship, generosity, and a shared commitment to a world without Alzheimer's. No pictures... and that's how I know we were having fun! 🍦

Yesterday, Graceful Transitions and Focus Home Care sponsored an Ice Cream Social at the Colchester Senior Center, and what a sweet afternoon it was!

A huge thank you to Jodi and the entire team for making us feel so welcome. Michele and I scooped more than 50 dishes of Bliss Ice Cream from Mel's Creamery while Alyssa and Nasi served up the hot fudge, fresh strawberries, whipped cream, and all the extras so everyone could create their perfect sundae.

The room was filled with smiles, laughter, and great conversations. We were having so much fun serving everyone that I completely forgot to take a single picture!

Thank you to everyone who spent a little time with us and shared some summer joy on a dreary day. Sometimes the sweetest moments are the ones that never make it to the camera. 💙🍦

Smoke from the Canadian wildfires is affecting the air we breathe, so today is a good day to take a few extra precautions. Older adults—especially those with heart or lung conditions—can be more sensitive to wildfire smoke.

And please take a moment to check on an older neighbor or loved one. Sometimes the smallest gesture makes the biggest difference. 💙

After months of planning we're proud to announce the launch of ACAP SECT–River Valley, our local chapter of ACAPcommunity, a national 501(c)(3) nonprofit serving Middlesex and New London Counties.

Graceful Transitions is honored to join 10 dedicated professionals who serve seniors throughout our community. Together, we're bringing trusted education, local resources, and support to the adult children who are navigating the many decisions that come with caring for an aging loved one.
