Location: Easton, MA | Schedule: Full-Time, Monday–Friday

Concordia Homecare & Nursing Services®️, a licensed and Medicare-certified home health agency is seeking a motivated Nurse Coordinator to support our clinical team and help deliver exceptional, compassionate care to our patients at home.

✨ Key Responsibilities:
• Coordinate start and management of patient care
• Support & supervise field RNs, LPNs, and HHAs
• Review OASIS, POCs, and skilled notes
• Lead case conferences & collaborate with physicians
• Monitor productivity and documentation compliance
• Participate in QAPI and staff onboarding
• Provide skilled nursing visits as needed

🌟 We’re Looking for:
• RN or LPN licensed in MA
• 2+ years home health experience (1+ year leadership preferred)
• Strong clinical judgment & documentation skills
