Let's Learn about Community Supported Living! Clearwater works with people in the community that need support due to developmental, physical or behavioral disabilities. These disabilities sometimes lead to challenges living independently.
Clearwater provides support to people with the following goals:
- Remain successful living in their own home or apartment
- They develop skills, strategies and plans to successfully live on their own
- Our members get to learn at their own pace
We work with three common populations of individuals:
1. Individuals that need the tools, resources and community access to learn how to live independently. For example, a person may be moving into their own apartment for the first time on their own. They keep forgetting to take their medications and forgot about their medical appointment with their doctor. Clearwater may help the person learn how to remember medications daily and help implement a system to track their appointments so they don't forget.
2. Individuals that have had health and wellness changes and need help learning to adapt to those changes. For example, if someone's health has declined and they struggle to stand for long periods of time, Clearwater may assist this person with learning how to complete basic household chores differently, adjusting to their current condition and strengths.
3. Individuals that would like to remain in their own home or apartment instead of long-term care options such as assisted living. Clearwater can provide full support with hygiene, personal cares, transportation, cleaning, errands, cooking, medication reminders, grocery shopping and more so that the member is able to remain in their current setting with support in place.
