This is truly disrespectful, and quite alarming! This is not an easy career. I know so much more now after having been in this field for 36 years, but I get paid less and less every year.

For more than 20 years, Medicare reimbursement for speech-language pathology treatment has essentially stood still while the cost of providing that care has continued to rise.

In 2003, Medicare reimbursement for speech, language, and voice therapy was approximately $77 per session. In 2026, reimbursement for that same treatment code is approximately $76 per session.

During those 23 years, general prices have increased by approximately 82%.

Rent is higher. Utilities are higher. Liability insurance is higher. Equipment, technology, supplies, and virtually every other expense required to operate a healthcare practice cost significantly more.

And, importantly, we have continued to increase what we pay our clinicians and staff over the years despite never receiving an increase in reimbursement along with rising expenses. Our employees should not have to absorb the effects of stagnant reimbursement through stagnant wages simply because Medicare has failed to keep reimbursement aligned with the rising cost of providing care.

Yet while the cost of operating a clinic and appropriately compensating highly trained healthcare professionals has continued to increase, Medicare reimbursement for this service is actually slightly lower today than it was 23 years ago.

Now, beginning January 1, 2027, the current treatment code is being replaced with new timed codes. Under Medicare’s proposed 2027 rates, the new code for 30 minutes of voice therapy that we typically provide would now reimburse approximately $55.

This is why reimbursement matters. It isn’t simply about what a healthcare provider gets paid. It affects whether practices can sustainably employ skilled clinicians, provide adequate treatment time, invest in specialized services and equipment, and ultimately continue making these services available to patients.

Patients deserve continued access to high-quality speech, language, cognitive, and voice therapy. Reimbursement must reflect the real cost of providing that care.

There’s immediate and delayed echolalia. If your child says some “phrases” or “chunks” clearly, but other comments are unintelligible…..could the clear phrase be delayed echolalia? YES!

Bringing this old post back because it asks such an important question: How do you advocate for less compliance and more regulation--especially when you feel like you are the only one in the room that believes in this imperative shift?

I was recently asked to help with a student---according to the referral he is a 17 year old autistic student who is currently "struggling" at school. He has been suspended multiple times this year for "aggressive and inappropriate behavior". Student has an IEP with Social Skills Goals and a recent FBA w/ updated Behavior Plan/Goals. Zero related services like speech or OT. Zero mention of supports for regulation, sensory, communication, predictability. Zero curiosity for his inner experience!!! When he becomes "aggressive" the plan is to get him to the "quiet room" which is a stark white room with nothing in it ("for safety of the student and staff" they say). If he continues to escalate and do things that “break school rules” he gets suspended. Every question I asked was met with a 'we tried that' or 'that would reinforce his behavior' or 'he needs to learn that this behavior is illegal, inappropriate, etc'. I felt so alone and activated in this meeting, but couldn’t stop thinking about how many million times worse this student might feel coming to school every day.

October is Dysautonomia Awareness Month!

Dysautonomia, a condition that affects the autonomic nervous system, is common among people with Ehlers-Danlos syndromes (EDS), particularly hypermobile EDS (hEDS), and hypermobility spectrum disorders.

The most common forms include orthostatic hypotension, PoTS, and vasovagal syncope. Symptoms can include:

- Dizziness or fainting
- Rapid heartbeat
- Fatigue and headaches
- Exercise intolerance
- Blurred vision or nausea

Management often includes lifestyle changes, such as increasing fluid intake, dietary adjustments, and learning postural maneuvers. These steps can help improve your quality of life. Learn more from expert Dr. Mark Belham here: https://www.ehlers-danlos.org/information/understanding-pots-webinar/
