About Us
We are a rapidly growing, AODE‑accredited outpatient dual‑diagnosis clinic serving the entire state of Kentucky through Telehealth, as well as Hardin County and the surrounding Bluegrass region where we offer Hybrid services, either Telehealth or In Office visits. Our multidisciplinary team—including licensed therapists, clinical social workers, targeted case managers, and supported employment specialists—provides evidence‑based, whole‑person care designed to promote long‑term stability and sustainable recovery.
Position Overview
We are seeking Four compassionate and dedicated Advanced Practice Registered Nurse Practitioners(APRNS) to join our Elizabethtown office. In this outpatient role, you will assess, diagnose, and treat Mental Health Condition s and Substance Use Disorders. You will provide comprehensive medical evaluations, manage Suboxone/Buprenorphine inductions and maintenance, and collaborate with our counseling team to support patients overcoming opioid use disorder and managing Severe Mental Illness.
Key Responsibilities
Conduct initial history and physical (H&P) exams for patients seeking substance use disorder treatment.
Prescribe and manage buprenorphine/Suboxone products, utilizing local protocol guidelines.
Provide ongoing medical evaluations, medication adjustments, and monitoring of patient compliance.
Psychiatric Evaluations: Conducting comprehensive mental health and psychosocial assessments.
Medication Management: Prescribing and adjusting psychiatric medications, as well as monitoring them for efficacy and side effects.
Psychotherapy: Providing individual, group, or family therapy to help patients develop coping mechanisms.
Crisis Intervention: Delivering emergency mental health care or stabilizing acute mental health crises.
Care Coordination: Collaborating with primary care physicians, therapists, and social workers to ensure cohesive treatment.
Collaborate with on-site therapists and licensed clinical social workers, to address co-occurring mental health challenges.
Maintain accurate electronic medical records (EMR) in compliance with Kentucky state regulations and DEA standards.
Qualifications & Requirements
Education: Master’s or Doctorate degree from an accredited Nursing Practitioner program.
Licensure: Active, unencumbered APRN license in the Commonwealth of Kentucky (adult care certification preferred).
Certifications: Active DEA registration and valid DATA 2000 waiver (or willingness to promptly obtain buprenorphine prescribing certification/X-waiver training).
Experience: Minimum 1 to 2 years of clinical experience in addiction medicine, substance abuse recovery, and/or psychiatric/mental health nursing.
Passion for community outreach and helping individuals rebuild their lives amidst the addiction crisis.
