Transitions offers the highest quality of end of life care, symptom management, and primary care services. We specialize in supporting individuals and families who are facing the physical, emotional, social, and spiritual challenges confronted when dealing with terminal and chronic illnesses. Paramount to our philosophy is to ensure that our patient is experiencing their final transition on their own specified terms — in comfort and with dignity. In our care, our main objective is always to maximize patients’ comfort and quality of life.

When someone you love is facing the end of life, the hardest questions often aren’t medical. Our chaplains are there for those moments, offering comfort, a listening ear, and support for people of every faith or none.

Join us this October for “Dementia Care: Fundamentals, Ethical Issues & Medical Needs,” a one-hour virtual presentation focused on building confidence in caring for individuals with dementia across healthcare settings.

This CEU will cover:
• Foundations of dementia care
• Ethical decision-making and patient autonomy
• Common medical needs
• Safe, evidence-informed, person-centered care

Choose from two sessions:
Tuesday, October 13 | 12:00–1:00 p.m.
Thursday, October 15 | 6:00–7:00 p.m.

CEU credit is available to Illinois nurses, and all healthcare professionals are welcome to attend.

This Labor Day, we’re grateful for the dedicated people who show up each day to care for others.

Ending summer with a little fun!

We had such a wonderful time celebrating the end of summer at Autumn Fields in Tilton! From smiles and laughter to carnival fun, it was a great day filled with special moments and memories.
