On this Memorial Day, we pause to honor and remember the brave men and women who made the ultimate sacrifice in service to our country. Their courage, sacrifice, and dedication to our nation will never be forgotten.

At Transitions Care, we are proud to support and care for Veterans through our partnership with We Honor Veterans. This program helps ensure Veterans receive compassionate, respectful care that honors both their service and their story.

Today, we reflect with gratitude for those we have lost and recognize the families who continue to carry their legacy forward. Wishing everyone a meaningful Memorial Day as we remember and honor those who gave everything for our freedom.

One of the biggest misconceptions about hospice is that patients have to leave home.

Hospice care can often be provided wherever a patient calls home, including:

- Private homes
- Assisted living communities
- Skilled nursing facilities

The goal is comfort, dignity, and support in the environment that feels most familiar.💜
