Near the forks of the Black River, Life Care Center of Elyria, Ohio, is less than 3 miles from Elyria Medical Center. Our nursing home is not just a place for 24-hour skilled nursing care for long-term residents but also a place for short-term inpatient and outpatient rehabilitation with physical, occupational and speech therapies. No matter the service offered, we always strive to ensure the highest level of quality to each patient.

🎉 We are so excited to announce that we recently earned a deficiency-free health survey!

✨ Surveyors from the health department spent time interviewing residents, family members, and associates. They evaluated resident care, recordkeeping, staffing levels, food safety, infection control practices and more.

“I am incredibly proud that we recently completed our first state survey in over 3 years and achieved a deficiency-free survey,” Staci Lehmkuhl, our Executive Director, shared. “The results reflect the commitment of our entire staff, who show up every single day with unmatched dedication, hard work and love for our residents.” 🫶

Deborah was very weak when she came to us for rehabilitation, but look at her smile now! 😃

When she started physical and occupational therapies, Deborah needed help with mobility, balance, and self-care skills. She worked hard and made significant progress. She said, "The therapists were very encouraging, bright, open and pleasant. My daughter, Stephanie, is a therapist and works here. We both knew that I would get great care! I truly appreciate everything they did for me!" 💬

At discharge, Deborah had achieved every therapy goal. She returned home safely and independently. Congratulations, Deborah! 🎉
