Does your child need a vaccination before starting back to school?
Students are not allowed to begin school without:
*proof of required vaccination/s
OR
*signed and notarized immunization affidavit exemption form
OR
*medical immunization exemption form signed by child’s medical provider
(this is only provided in specific circumstances)

It is your right as the parent which you will provide, but either of the above will be required for your child to start school.
