📢 Important Information Regarding Dr. Simon

As Eunice Health Clinic continues to grow, we are excited to welcome Dr. Simon to our provider team alongside Brian Henning & Candida Coburn. We look forward to expanding access to quality healthcare for our community.

Dr. Simon will begin seeing patients on August 10th!

To help provide the best possible care from your very first visit, we will be gathering medical records from your current provider before scheduling appointments.

If you are an existing patient of Dr. Simon and would like to establish care with him at Eunice Health Clinic:

✅ Call our office to complete a quick registration.

✅ We will send you a medical records request form.

✅ Let us know when your next follow-up appointment is due so we can help prioritize scheduling.

✅ Please call back at the beginning of August to schedule your appointment.

Dr. Simon will begin training with our team in July, and we look forward to welcoming him to the clinic in August!

📌 Dr. Simon will be accepting new patients, with priority given to Eunice-area patients first, followed by scheduling as availability allows.
