People with ADHD experience prolonged nervous system overwhelm due to a combination of structural brain differences and intense emotional processing known as Rejection Sensitive Dysphoria (RSD). In an ADHD brain, the prefrontal cortex—the area responsible for regulating emotions and filtering stimuli—has lower connectivity with the amygdala, which acts as the brain’s emotional alarm system. When faced with a harsh comment, a sudden disruption, or an intense conversation, the amygdala fires rapidly, treating the psychological threat exactly like a physical danger. Because the executive brake system is weak, the brain cannot easily dampen this signal. This triggers a massive, immediate flood of stress hormones like cortisol and adrenaline, locking the individual into a visceral, full body fight-or-flight response that they cannot simply rationalize away.

Once this nervous system cascade begins, it can take hours to subside because of a phenomenon called emotional hyperarousal and impaired neurotransmitter regulation. While a neurotypical nervous system might process a sudden change and return to a baseline state within minutes, the ADHD nervous system lacks efficient dopamine and norepinephrine transport to smoothly transition between emotional states. The initial shock becomes a feedback loop where the body remains physically agitated—manifesting as a racing heart, shallow breathing, or intense muscle tension—long after the triggering event has passed. Because the brain struggles to shift its attentional focus, it continuously loops the distressing event, keeping the stress response active and preventing the parasympathetic nervous system from stepping in to calm the body down.
