NOW HIRING – SOCIAL WORKER!
Greene County Health System is seeking a compassionate, dependable, and dedicated Social Worker to join our healthcare team!
This position will provide social services support to patients and residents throughout our health system, including both hospital, physician's clinic and long-term care settings.
Key Responsibilities:
Conduct psychosocial assessments and develop individualized care plans.
Coordinate discharge planning and post-discharge services.
Assist patients, residents, and families with accessing community resources.
Participate in interdisciplinary care planning.
Provide emotional support, advocacy, and assistance with patient and resident needs.
Maintain accurate documentation and ensure compliance with healthcare regulations.
Qualifications:
Degree in Social Work or a related field.
Healthcare or long-term care experience preferred.
Excellent communication, organizational, and interpersonal skills.
Knowledge of discharge planning, patient advocacy, and community resources.
Must meet applicable Alabama licensure and regulatory requirements.
