Navigating the DDD system can feel overwhelming, especially when you are already focused on caring for a loved one.

At Your Choice Home Care, our goal is to make the process as simple and stress-free as possible. From helping families understand eligibility requirements to guiding them through onboarding and coordinating appointments, we are here to help every step of the way.

We believe families should spend less time worrying about paperwork and processes and more time focusing on what matters most: their loved ones.

Support should feel personal, accessible, and compassionate, and that is exactly what we strive to provide.

No two individuals are exactly alike, which is why care should never be one-size-fits-all.

Every person has their own strengths, challenges, goals, routines, and preferences. Personalized care recognizes those differences and focuses on providing support that reflects each individual's unique needs.

When care is tailored to the person, it can help build confidence, encourage independence, and improve overall quality of life. It also helps families feel more supported, knowing their loved one is receiving care that is designed specifically for them.

At the heart of personalized care is something simple but powerful: treating people as individuals, not just a diagnosis.

There is something special about care that comes from someone who already knows and loves you.

Family caregivers often understand routines, preferences, communication styles, and individual needs in a way that creates comfort and consistency. That familiarity can make a meaningful difference in the daily lives of individuals with developmental disabilities.

Family caregiving is about more than assistance with daily tasks. It is about trust, connection, and providing support from someone who genuinely cares.

When families have access to the right resources and support, they can continue providing high-quality care while maintaining stability for themselves and their loved ones.

What Happens After You're Approved for DDD?

Getting approved for DDD services is a major milestone, but many families are unsure what comes next.

After approval, the next steps often include completing or reviewing assessments like the NJCAT, which stands for the New Jersey Comprehensive Assessment Tool. Families may also select a Support Coordination Agency and begin developing an Individualized Service Plan, also known as an ISP.

These steps help identify the individual’s goals, needs, level of support, and services that may be available.

From there, families can begin exploring the right services and supports, including options that may allow a family member or loved one to become a paid caregiver.

Every journey looks a little different, but DDD approval is an important step toward accessing resources that can improve quality of life, support independence, and provide meaningful help for families.

Congratulations to our good friend Captain Jason Gerba of the Edison Police Department on the incredible article recognizing his work with Special Olympics New Jersey and the Law Enforcement Torch Run.

Captain Gerba has been a true leader in using his role to create safer, more inclusive communities. From his ongoing commitment to Special Olympics to his leadership with programs like the Blue Envelope Program, his work continues to make a meaningful impact for individuals with autism, individuals with intellectual and developmental disabilities, families, athletes, and law enforcement across New Jersey.

Your Choice Home Care is proud to celebrate this well-deserved recognition and grateful for the work Captain Gerba continues to do.

Congratulations, Captain Gerba!

After nearly 30 years supporting the cause, Edison Police Capt. Jason Gerba will carry the Flame of Hope to open the NJ Special Olympics Summer Games.

One of the most common questions we hear is: "Where do I start?"

Navigating the DDD system can feel overwhelming, especially when families are trying to understand eligibility requirements, assessments, and available services.

The good news is that you do not have to figure it out alone. Learning about DDD, Medicaid eligibility, and the NJCAT is the first step toward accessing meaningful support and services for your loved one.

Knowledge is empowering, and understanding your options can make the journey feel much more manageable.

June is National Safety Month, a reminder that creating safe and supportive environments can make a meaningful difference in everyday life.

For individuals with developmental disabilities, safety looks different for everyone. It may mean maintaining routines, creating sensory-friendly spaces, improving accessibility, or making sure support systems are in place.

Small changes can have a big impact on independence, confidence, and overall well-being. This month, we encourage families to take a moment to evaluate their environments and celebrate the steps they have taken to create safe, supportive spaces for their loved ones.

Wondering if your loved one qualifies for DDD services in New Jersey?

To qualify for services through the Division of Developmental Disabilities (DDD), your loved one must live in New Jersey, be over the age of 21 to receive services, and have a chronic intellectual or developmental disability that began before the age of 22. Individuals must also be Medicaid eligible in order to access DDD-funded services.

For many families, the process can feel confusing or overwhelming at first, especially when trying to understand eligibility requirements, paperwork, and next steps. The good news is that support and guidance are available.

Understanding the qualifications is an important first step toward accessing meaningful resources, personalized support, and services designed to help individuals with developmental disabilities thrive.
