Think of your Support Coordinator as your guide through the DDD system. They work with individuals and their families to understand their goals, identify their needs, and connect them with the services and supports that can help them thrive.

A Support Coordinator can help:
✔ Develop your Individualized Service Plan (ISP)
✔ Connect you with approved service providers
✔ Answer questions about available services and supports
✔ Monitor your plan and make updates as your needs change
✔ Advocate for services that support your goals and independence

Your Support Coordinator plays an important role, but you don't have to navigate everything on your own.
