An ear-to-ear smile says it all 😊

Pedro’s first fitting of his palatal-drop prosthesis connected to a zirconia full-arch restoration was a truly special moment. All the steps of this process finally came together beautifully. And Pedro quickly learned to place and remove the prosthesis using a PUL tool , gaining confidence right away. And even more excitement as he felt how naturally his tongue could once again reach the roof of his mouth. Speech, function, confidence… all coming back together.

Seeing his joy reminds us why we do what we do. When patients like Pedro leave smiling this big, it’s more than a restoration— it’s a return to living fully.

After tongue cancer surgery, everyday things like speaking and eating can become really difficult. In this case, I created a custom palatal drop prosthesis that brings the roof of the mouth down just enough so the remaining tongue can make contact again. It allows the patient to clear food more effectively and speak more clearly, which can make a meaningful difference in daily life.

I also want to thank my dear friend and colleague Dr. Anthony Sallustio , whose maxillofacial case presented sparked the idea for this design: Fixed teeth with a removable palate. And to the legendary Cory Glenn , a true pioneer in implantology, digital dentistry, and dental CAD. I’m grateful that we brainstormed the design of this case together, especially around the concept of milling semi-precision attachments in zirconia.

Part 2: Palatal-Drop Prosthesis - Housing Pickup Technique. I demonstrate how to fit a 3D printed palatal drop prosthesis over Sagix housings on a full arch zirconia restoration. We confirm passive seating, verify the intimate fit against the teeth, and then pick up the housings.

Palatal-Drop Prosthesis: Restoring Function Beyond the Smile
A really interesting full-arch treatment featuring a monolithic zirconia bridge on four implants—with an important functional addition. My patient is a tongue cancer survivor who underwent a glossectomy (tongue removal), leaving the remaining portion of his tongue unable to contact the roof of his mouth or his teeth. That gap—or ā€œblind spotā€ā€”creates real challenges with both speech and food clearance.
To address this, we’re designing a snap-in palatal drop prosthesis. By lowering the palatal contour, the remaining part of the tongue can function again—helping sweep food away and improve speech.

- Loss of 85% of tongue due to cancer surgery
- Inability of residual tongue to contact roof of mouth
- Food trapping in palatal/roof space
- Impaired speech articulation
- Functional limitations despite a fixed full-arch restoration

- Minimize food entrapment in the palatal vault space

- Maintain a stable, fixed full-arch implant restoration
- Incorporate four semi-precision palatal attachments for retention

- Ultimately improve day-to-day function and quality of life

Final insert. Up to this point, we’ve showcased case planning and careful step-by-step ex*****on. In this final video, we see the final restoration featuring monolithic milled zirconia with ovate pontics and custom zirconia abutments. We examine the precise seating of the restoration and highlight the beautiful papilla formation on the facial aspect near the gingiva.

While screw-retained restorations remain our first choice whenever possible, there are situations where cement-retained restorations are necessary. In those cases, it must be done correctly—using properly designed custom abutments with equigingival margins to prevent subgingival cement extrusion. When executed properly, the result is healthy bone, healthy gingiva, and ultimately a happy patient and restoration with an excellent long-term prognosis.

Milled Zirconia Bridge Test Seating and Occlusal Adjustments. We demonstrate the seating of custom zirconia abutments followed by verification of the milled zirconia cementable bridge. We perform occlusal adjustments and verify proper gingival adaptation of the ovate pontic sites. After confirming perfect fit and function, the zirconia bridge is ready for stain and glaze finishing, and a temporary printed restoration of the final restoration is seated until final cementation.

Part 3. Custom Abutment & Cementable Restoration: Digital Design to Manufactured Reality.

Showcasing here the manufactured restoration process following the digital design, featuring the TiBase custom abutment, and final restoration. TiBases, selected specifically for this case with a 2.5mm gingival height due to necessary deeper implant placement. We demonstrate how the solid model confirms passive seating on the custom abutments and serves as a verification tool post-glaze. Before glazing, we perform a complete try-in of both custom abutments and the four-unit cementable bridge, adjusting occlusion and contacts to ensure optimal fit without post-glaze adjustments.

Part 2: CAD/CAM. In the prior post I showed the digital CAD design. Now let’s take it out of the mill and see the CAM product!

This is a custom designed case featuring a four unit bridge and two custom abutments. In this clip we look at the milled restoration, check the fit of the components, and get everything ready for green state hand finishing before it goes into the sintering oven.

Stay tuned for the next steps after sintering and the try in.
