preventative and restorative dental care, oral surgery, mental & behavioral health services for children and adults, Psychiatry, Health Insurance Enrollment Assistance, substance use disorder treatment, and specialty services including optometry eye care, optical shops, nutrition consultations, podiatry, TMS Clinic, Spravato Clinic and Outpatient Day Treatment. CHC has five sites within Fitchburg, Gardner and Leominster with decades of experience as a Federally Qualified Health Center (FQHC), serving 36 communities in North Central Massachusetts. All CHC health centers are located on the Montachusett Regional Transit Authority bus line. Community Health Connections was established in 2002 and is a Federally Qualified Health Center (FQHC). Wonderful morning with students from Gardner High School! It was a pleasure to have them, and our staff loved the opportunity to share their clinical and non-clinical healthcare career paths. 😍 NC

Thank you to the South Gardner Community Health Center for meeting with a group of our 10th and 11th grade students interested in the healthcare field. Students got a tour and were able to hear from nurses, radiologists, managers, etc. to learn about many different job possibilities. It was great information for our students!

Proud moment! Here is a photo from Narragansett Regional School District Templeton/Phillipston, MA Narragansett Middle School National Junior Honor Society induction ceremony featuring Jonah Jovanovic (left) and Andrew Richard (right). Both students completed their community service hours at the Greater Gardner Community Health Center, where they worked alongside our team and gained hands-on experience. They learned how to check blood sugar and blood pressure, stocked and cleaned exam rooms after patient care, checked expiration dates on supplies, practiced proper glove techniques, and helped front desk staff sort and file faxes and paperwork. They both did a wonderful job, learned so much, and were a pleasure to have with us. - Shawna Jovanovic, LPN, Nurse Manager, CHC 👏👏👏👏 A big thank you to Andrew and Jonah and congratulations on this well-deserved honor! 😎

Thanks to additional funding CHC received through the Linkages to Care Grant from RIZE Massachusetts Foundation CHC will continue its important work alongside our local hospital partners, supporting individuals who are dealing with substance use.

Linkages to Care funding helps bridge the gap between crisis and care, allowing our Certified Addiction Recovery Coaches to meet patients in our local emergency departments, bedside, or in home settings-providing compassionate support and assistance with connection to treatment, recovery resources and hope.

It allows us to build upon the strong foundation, partnerships and connections we have established over the past few years and will assist us in continuing and expanding this lifesaving work throughout our communities here in north central Massachusetts.
