The completed AD-3027 form or letter must be submitted to USDA by:

We just wanted to remind everyone that the Clay County Health Department will be closed tomorrow April 18th 2025 for Good Friday. If you need us for anything we are open today until 5pm and will reopen on Monday at 8am. We hope everyone has a safe and Happy Easter!
