Does HBOT help a person with stroke recovery? It absolutely can!

Whether you are using Hyperbaric Oxygen Therapy (HBOT) or not for stroke recovery, the goal we have as providers post stroke is to help a patient regain as much function as possible.

In order to heal and regain function, we have to work to help shrink the penumbra (area of dormancy) down to the smallest amount of space possible.

If you are using HBOT post stroke in addition to PT, OT, or other exercise related therapies, starting with HBOT can help to oxygenate the area and start to wake it up.

Then, we can revisit PT and OT to work on fine motor and gross motor skills. That way we can decrease spasticity, improve speech, and/or improve motor control.

So, we wouldn't be doing hyperbaric INSTEAD of those other therapies.

At one point, we actually want to be doing HBOT in addition to those therapies to really get the most out of both worlds.

If you incorporate HBOT with PT and OT therapies, you should actually start to get another round of improvements from a lot of the different types of issues that occurred from the stroke.

How are EWOT and HBOT different? Or are they comparable?

Is EWOT an oxygen delivery therapy? Absolutely.

EWOT stands for exercise with oxygen therapy.

You're creating a nice varied supply and demand type of equation, and using that you can most certainly deliver oxygen to the working tissues at higher levels because of manipulation of the supply and demand systems.

However EWOT cannot deliver oxygen to other tissues like Hyperbaric Oxygen Therapy (HBOT) can.

HBOT is a passive therapy. EWOT is an active therapy.

You're exercising with EWOT vs. literally laying down and passively relaxing during hyperbaric oxygen therapy

Why?

Because when you're exposed to hyperbaric conditions and you're being passive during that process, it just gives your body an opportunity to create a large surplus of oxygen and use it anywhere it needs it.

You're not necessarily actively metabolizing all of that oxygen in that moment and you can create a large surplus while you’re in the hyperbaric chamber.

EWOT is relying 100% on red blood cell delivery. With hyperbaric, you can certainly saturate red blood cells as well, but it's also delivering an enormous amount of oxygen into the plasma.

This super saturates the body with far more oxygen than EWOT could ever do.

So, if the concern is getting oxygen into the body for healing, recovery, and regeneration, hands down, HBOT is going to deliver more oxygen and give the body a chance to utilize that oxygen because the body is resting passively versus exercising actively.

How long do the benefits of Hyperbaric Oxygen Therapy (HBOT) last?

This is one of the TOP questions we get about HBOT.

If there was an injury, a trauma, a concussion, a TBI, or a surgery and we're looking to heal THAT thing, the treatment can certainly have a beginning and an end.

Once they get the healing benefits they were looking for, they should keep them forever once they stop.

However, somebody else might be coming in with a degenerative disease, autoimmune disease, or a neurodegenerative disease that doesn't necessarily have a beginning and an end.

In those cases, I believe that if we're getting benefits and changes that we're looking for, the treatment may not have a definitive end.

Which means that we might do a certain intensity of therapy, initially, to get some of the initial responses that we're looking for. Then, if the initial protocol shows a lot of promise and we're getting a lot of those benefits, we are likely to continue using HBOT to some degree over a long period of time, maybe forever.

Although the intensity, duration, and frequency that we started with is probably not going to be what we continue with…we’d likely switch to some sort of maintenance program.

Usually it's much more difficult to create the changes and much more easy to maintain those changes over the long haul. So, the frequency and duration should go down in a maintenance mode as compared to an active care mode.

I think that's really important to explain to people.

If their condition or disease isn't going away, but this therapy is helping them, then they’ll likely want to keep using this therapy long term.

Hyperbaric Oxygen Therapy (HBOT) might be working...just not on what you expected first.

Many people start hyperbaric oxygen therapy with a list of things they're hoping it will fix. The frustrating reality? You don't get to choose which benefit comes first.

HBOT creates universal cellular changes in everyone, but how those changes show up in your life varies. Your body decides what to heal first, not you.

If you're seeing ANY positive changes (better sleep, more energy, improved mental clarity, stronger immune function), you're on the right track. Those subtle signs mean the therapy is working, even if your main symptom hasn't improved yet.

Keep going. The benefit you're waiting for will likely break through if you hold the course.

Here’s how to calculate the dose of oxygen that someone is getting in a hyperbaric oxygen therapy (HBOT) session.

You take the atmospheric pressure times the percentage of oxygen times the number of minutes, and the duration and frequency of the session.

With that calculation, we could effectively create a dose of oxygen that this person received.

Now, there are certain benefits of HBOT that do include the specific dose of hyperbaric oxygen.

Goals like, increased mitochondrial efficiency, increased immune function, and reduced inflammation.

These few things are absolutely affected by the total dosage of oxygen as well as a few others.

Lastly, maximizing the oxidative component of the therapy, which in some cases maximizing the oxidation is one of the benefits.

So, if we’re talking about mitochondrial function, immune function, inflammatory results, and oxidation, the total amount of oxygen that we get into somebody’s body is a very meaningful conversation.

In those cases, the total dose of oxygen is important.

There's not a lot of extensive research on all the different types of pressures that could be used with Hyperbaric Oxygen Therapy (HBOT) for ALL the different types of patients and issues they could be suffering with.

So we need to understand what research is out there and try to extract the best information we can.

There was a study done on a pressure of 2.0 atmosphere. They did 60 sessions and they saw an eight times increase in mesenchymal stem cells and central nervous system stem cells.

Then recently, there was a paper done with almost the exact same metrics that the 2.0 pressure study did, but they used a soft chamber, 1.3 pressure and air only. They found somewhere between a 2.5-3 times increase in those same stem cells. So at that lower pressure, we saw a stem cell response as well, it was just lower.

Can we continue to stimulate that over a longer period of time to get something more similar to, let's say that eight times increase in two months? The real answer is we don't know, but it's reasonable to believe that might be true.

Autophagy comes up a lot in the health and longevity space, specifically around fasting.

Your body is made up of trillions of cells and those cells are constantly being replaced by new cells.

There are two main mechanisms of that cell turnover:

Autophagy: cellular recycling

Apoptosis: cell death.

Autophagy is a way of breaking cells down, or breaking certain organelles or systems down inside of the cell, and replacing them with new organelles, or replacing that entire cell with stem cells.

This improves the youth and the vitality of those cells inside of whatever tissue it is that we're talking about. The strongest stimulant to autophagy is cellular adversity. Fasting creates cellular adversity.

By not relying on food for energy, your body starts to look for stored energy as a source for fuel, and as a result, the body has to go into storage and actually start breaking down cells and releasing stored energy.

Hyperbaric Oxygen Therapy (HBOT) also helps upregulate autophagy.

There have been a number of studies looking at how HBOT may help stimulate or increase this autophagy pathway.

So, combining fasting with HBOT can be an incredible way to really draw out the benefits of both and really push that autophagy pathway much harder than either one can do alone.

The interesting thing about HBOT is there are studies that show evidence that it helps stimulate further autophagy pathways. At the same time, we also know that it's a therapy that stimulates the growth, repair, and regeneration of tissue. It’s a powerful therapy with many benefits, including stimulating autophagy.
