Sunrise PACE

Sunrise PACE PACE is a Program of All-inclusive Care for the Elderly, allowing participants to age in place in the community.

The PACE approach
Your care is provided by an Interdisciplinary Team (IDT) that is assigned to each participant consisting of the following staff:

• Physician
• Registered Nurse
• Home Care Coordinator
• Masters level Social Worker
• Occupational Therapist
• Physical Therapist
• Recreational Therapist
• Registered Dietitian
• PACE Center Manager
• Certified Nurse Assistant
• Transportation Driver



The IDT assesses each participant’s needs and delivers services based on a customized care plan. Where needed, the plan will include acute care, hospitalization and nursing facility.

Did you know that up to half of falls among older adults are caused by hazards at home? Everyday items like clutter, loose rugs, poor lighting and slippery floors can increase your risk.
