About 20 years ago, a mother once brought her 7-year-old son to see me. Up until a couple of months before they came to my office, her boy was generally a well-behaved, pleasant child, but his behavior had gotten progressively worse. The slightest thing would set off a tantrum, causing him to lay on the floor, screaming. She'd already taken her boy to a psychologist, and they couldn’t figure out what was going on.

I asked him a few questions, and he mentioned that his legs hurt at night so bad that he had to get up and walk around.

That caught my attention. I asked him, "Like a charley horse?"

He didn't know what I meant, but I had an idea!

I ordered a hair mineral analysis; when the results came back, his magnesium wasn't just low. The lab's equipment couldn't even measure it.

With food and a supplement, we built his magnesium levels back up until it finally reached a normal range and the symptoms disappeared.

Thyroid specialists often miss a perimenopause-thyroid overlap. They may be highly skilled, but they might not see the holistic picture.

Often, a specialist looks at thyroid numbers, sees they're technically in range, and says it's not the problem … but the bigger picture is often missed.

If you're being told your numbers are in range, but it doesn't match how you feel, ask specifically for T3 and reverse T3 to be tested, not just TSH and T4. That's usually the next step in finding the answer, or at least ruling-out low thyroid function.

When a client doesn't have recent bloodwork, I often use a questionnaire covering 38 possible symptoms, graded by level of severity. It's anecdotal, not hard science. But it's often nearly as useful as a blood test, sometimes more, because nobody knows how a woman feels better than she does.

After decades of doing this work, I've come to believe the most impactful thing I can do for a client isn't a test. It's listening.

New on the blog: Why perimenopause throws thyroid function off, whether it’s never been a problem before or you’re already being treated for it. Three things worth knowing:

-Your thyroid hormones can test perfectly normal and still not be doing their job, because the reproductive hormones that help your body use them have dropped.
-Hair loss and hair getting coarse are two different symptoms with two different causes, and only one of them is primarily about your thyroid.
-Testing TSH and T4 alone isn’t always conclusive. A full panel catches what a single test regularly misses.

Doctors are trained about drugs and surgery in med school, but less about the day-to-day physiology of a hormone system in transition. A single blood test on a calm Thursday captures just one snapshot of a month’s worth of changes, but it's seen as the whole story.

I have many clients right now who are in their 30s and aren’t having regular periods. I always go back to the liver to see how liver support will help.

The ovaries make the hormones but the liver manages them. So when your ovaries begin to slow their hormone output, which they do gradually as you move through your late 30s and 40s, your body sends more raw cholesterol out looking for a home. If your liver isn’t converting it efficiently, or if you don’t have a gallbladder to help process the bile that makes that conversion possible, that cholesterol doesn’t disappear — it circulates. Your cholesterol numbers soar on a blood test, and your doctor reaches for a statin prescription. But that’s not addressing the actual cause.
