❌ We do not use the same type of estim current on every patient for every condition.

We have different settings to optimize specific outcomes.

Below are the most common settings we use in the clinic.

🔹Purpose: pain relief, reduce muscle guarding:
🔸Setting: Fast/slow
🔺Frequency: 2/6 hz or 2/8 hz

🔹Purpose: Muscle activation
🔸Setting: burst cycles
🔺Frequency: 2/6 hz, 3/10 hz, or 6/20 hz

🔹Purpose: Fluid dynamics/ muscle recovery
🔸Setting: continuous
🔺Frequency: 2-4hz

⚠️Dry needling is always paired with movement for optimal results.

💪 rehab based exercise programming can have a variety of goals: muscle activation, neuromuscular reeducation, movement pattern retraining, or muscle hypertrophy

✨For any goal, the dosage must respect the patient’s irritability - how sensitive or reactive a condition is to movement, load, or activity.

We assess irritability by looking at:

🔺Pain intensity → how severe the pain becomes
🔺Load tolerance → what intensity, duration, or volume of activity aggravates symptoms
🔺Symptom persistence → how long symptoms last after being aggravated

For example:

🔺High irritability = severe pain, symptoms triggered easily with low levels of activity, and pain lingers for hours or days.
🔻Low irritability = milder pain, higher tolerance to activity, and symptoms calm down quickly

✨Understanding irritability helps guide the dosage of exercise prescription and treatment progression

At REVIVE, we don’t just give you a print out of an exercise list and tell you to do 3 sets of 10 for everything. We adjust the dosage as your symptoms improve, your load tolerance increases, and your goals progress.

⚠️Precautions we take to avoid puncturing the lung field when dry needling the upper trap

1️⃣ Patient positioning
🔹We like to needle the upper trap with the patient laying on their stomach so the muscle is on slack and more pliable. We place the arms in specific positions or use bolsters to lift the target muscle away from the lung field

2️⃣Needle length
🔹We use 30-40mm needles in this area to reduce the possibility of going too deep into the lung field

3️⃣Angle of insertion
🔹We never insert perpendicular over the lung field. Even if we are superior the lung field, we angle the needle up towards the head

4️⃣ Gripping tissue
🔹We lift and separate the muscle away from the lung field and maintain a tight grip on the motor band we are needling while fanning the needle and during estim.

4️⃣ Hand held estim
🔹We only use the pointer Estim unit while maintaining our tissue grip to ensure the needle doesn’t change direction and puncture the lung field.

‼️Image findings are poor indicators of pain.

A large systematic review (Brinjikji et al., 2015) analyzed spinal imaging from 3,110 Asymptomatic individuals (people with NO PAIN)

The study found:

🔸disc degeneration in: 37% of people age 20, 68% of people age 40, 88% people age 60

🔸disc bulge in: 30% of people age 20, 55% of people age 40, 74% people age 60

🔸disc herniation in 29% of people age 20, 33% of people age 40, 38% people age 60


🚨Your neck pain most likely is NOT coming from whatever abnormality your doc pointed out on an MRI.

⚠️structural findings do matter when there is:
🔸symptomatic nerve root compression
🔸neurolgical defificts
🔸spinal cord compression with myelopathy

The basics are only “basic” when they’re prescribed poorly.

The obsession with flashy rehab exercises usually comes from ineffective programming, ❌ not because the fundamentals stopped working.

Good rehab doesn’t need to look complicated to create great outcomes.

At our clinic, we don’t chase novelty for the sake of creativity.

We focus on:
• proper dosing
• intentional progression
• quality movement
• consistency

When you understand how to program and prescribe correctly, the basics work exceptionally well.

❌ Stop stretching after a massage and do THIS instead

To effectively increase neck range of motion we have to teach the muscles HOW to use this new range — or else they will go right back to where they are comfortable being short and stiff 👎

After doing manual work (massage, joint mobilizations, dry needling) I do a sequence of muscle retraining techniques
1️⃣ active assisted motion through full range (I move your head but you help me)

2️⃣ end range isometrics (I give you manual resistance at end range and don’t let you move)

3️⃣ full range eccentrics (I give you manual resistance at end range and allow you to slowly move back to starting position)

4️⃣ full range active motion

This gradually teaches the muscles how to contract and relax in the new range.

These exercises are added to patient home program for lasting results.
