Brief Summary:
A 72-year-old woman had two spontaneous bleeding episodes from a varicose vein at her left medial ankle in the same month — the second sent her to the ER, though the bleeding had stopped by the time she arrived. She’d had large, longstanding varicose veins in her calf for years, and was previously told to “leave them alone until they bother you.” They didn’t bother her — until they bled. This case makes the point plainly: varicose veins are a visible sign of underlying venous insufficiency, fully mappable with ultrasound and treatable with safe, effective, well-tolerated endovenous ablation. Waiting for pain or bleeding is outdated advice. Every patient with varicose veins deserves an evaluation — ideally by a vein specialist with vascular surgical training. Don’t wait for your veins to bleed. Get evaluated proactively, not reactively.

“She came in for her leg. She left with a pelvic diagnosis that explained everything.”A 53-year-old nurse came in concerned about her left leg — swelling, heaviness, and skin changes below the knee near her calf and inside ankle. While waiting to be seen, she picked up an article on pelvic congestion syndrome and immediately recognized her own symptoms in it. She mentioned this to our nurse practitioner, who was sharp enough to order not just a venous insufficiency study, but an iliac duplex scan as well.

The insufficiency study showed great saphenous vein reflux — but it was actually worse on her right leg, the leg that looked normal. The iliac duplex told the real story: high-grade compression of her left common iliac vein, with pelvic varicose veins as a result of that blockage. Instead of moving straight to leg vein ablation, she was scheduled for an IVUS exam and possibly treatment with balloom angioplasty and venous stent.

The lesson: patients come to us because of what they see in their legs, but the legs don’t always tell the whole story. When something on an insufficiency study doesn’t add up, or a patient mentions something in passing, we owe it to them to dig deeper before reaching for the ablation catheter.

You don’t have to live with varicose veins, not anymore!

“You’re too young for vein disease… right? Think again.”
This young man came to me with brown discoloration around his ankle caused by stasis pigmentation—a sign of chronic venous insufficiency, not just a cosmetic issue. His ultrasound showed significant reflux in his great saphenous vein, allowing blood to pool in his leg day after day.
The good news? You don’t have to live with it.
Stasis pigmentation is often your body’s warning that vein disease is progressing. Left untreated, it can lead to worsening skin damage, inflammation, and even venous ulcers. But when caught early, we can often stop the progression and give your leg the chance to heal.
If your legs ache, swell, feel heavy, or you’re noticing brown discoloration around your ankles, don’t ignore it. Get evaluated. Vein disease is treatable, and there’s no reason to simply “put up with it.”
