Dr. Jacob Sambursky recently joined AdventHealth's nursing in-service day to walk through the updated AHA/ASA stroke guidelines, with a focus on extended window thrombolytics.

The best part was the room itself. Nurses drove the conversation with questions grounded in what they see at the bedside every day.

"Following through, and doing what you say you're going to do matters over here."

That is David Theroux, CEO of Hendry Regional Medical Center, a critical access hospital in rural Florida.

He is not describing a product. He is describing a standard.

In rural stroke care, the distance between a promise and a patient is very short. A team says it will be there. Then a patient arrives at 3 a.m. with symptoms that started forty minutes ago, and the promise is either real or it is not.

Stroke quality measures are nursing measures.

Door-to-needle time. Documentation completeness. Composite scores. Each one depends on a nurse who knows the next step and has specialist support to take it.

Hendry Regional Medical Center built its stroke program on that principle.

Median door-to-needle time fell from 63 minutes to 44 minutes. The GWTG-Stroke composite score rose from 74.9 percent to 95.1 percent. Both results were presented at ISC earlier this year.

Dakota Redd, CNO at Hendry Regional Medical Center, named what changed for the nursing team:

The 2026 AHA/ASA stroke guidelines made headlines for one reason: the treatment window extended to 24 hours. That headline is worth understanding in full.

The extension applies to specific patients under defined imaging and clinical criteria. For conventional window patients, the 4.5-hour standard remains unchanged — and a separate update to front-end imaging protocols means programs that have not reviewed their workflows may not yet be aligned with the current standard of care.
