Professional Nursing Home Health Care is seeking a compassionate, organized, and motivated Registered Nurse (RN) Case Manager to provide high-quality home health services to patients throughout the Fort Wayne area. Responsibilities
*Complete Start of Care (SOC), Recertification, Resumption of Care (ROC), Discharge, and supervisory visits.
*Perform comprehensive nursing assessments and develop individualized plans of care.
*Coordinate patient care with physicians, caregivers, therapists, and other healthcare professionals.
*Monitor patient progress and revise the plan of care as needed.
*Provide skilled nursing care, medication management, and disease process education.
*Accurately complete OASIS documentation and clinical notes within required timeframes.
*Supervise Home Health Aides and Licensed Practical Nurses as assigned.
*Participate in case conferences and quality improvement activities.
*Educate patients and families on disease management, medication safety, and home safety.
Qualifications
*Current, unrestricted Indiana Registered Nurse (RN) license.
*Minimum of one year of nursing experience (home health experience preferred).
*OASIS documentation experience preferred.
*Strong assessment, communication, and critical thinking skills.
*Ability to work independently and manage a flexible schedule.
*Reliable transportation, valid driver's license, and current auto insurance.
*Current CPR certification.
*Proficiency with electronic medical records (EMR) preferred.
