Meet Our Staff:

Monica Merritt, RN is our awesome Director of Nursing. She provides oversight of weekly nursing visits, maintains quality assurance, organizes continuing education for staff, and assures program integrity. Our RNs bring a passion for hospice service to the bedside with compassion, warmth and knowledge. They have extensive hospice experience and hearts for service. It is a blessing to have their experience and truly caring hospice perspectives on this team. Erika Clausen is our amazing LVN who attends to her patients with a true passion for hospice nursing. Her compassion and dedication is unrivaled. Our on-call team is exceptional and is comprised of tenured hospice nurses. Katie, Shonda and Dusty bring expertise, compassion and extraordinary attention to detail to all aspects of hospice nursing. Their kind and caring nature and love for hospice make them an invaluable resource to our clients for both day and evening calls. Tosha Williams, Jasmine Smith, and Princess Richardson are our phenomenal aides and are usually the patient's favorite part of the team. Their care and attention to detail keep patients feeling their best while maintaining the utmost dignity of the patient. Millie Hunt is our Marketing Manager and Community Liaison. She provides consultations to families, acts as a First Contact and has a plethora of information to help families make the best decisions for them when facing terminal illness. She is outstanding at explaining how hospice can work for you and your family. Her gentle approach and calm demeanor allow families to consider their options in a relaxed environment. Karen Taylor is our Intake Coordinator/Office Manager. She wears many hats and wears them well. She expertly manages the details of the referral/admission processes as well as some major compliance aspects. She has many years of hospice intake and office experience keeping all things running with precision behind the scenes. Our phenomenal Chaplains are available to staff and clients to provide support, comfort and care for all spiritual issues during a client's stay on hospice and after the journey has ended, for the family. They are friendly, warm, non-judgmental personalities, providing a non denominational listening ear and encouragement/support to our clients and their families (as well as our staff). Our Social Work teams are attentive to the needs of our clients and their loved ones. They can provide information and resources to help meet the needs of our families. They can also help with official forms and paperwork, end of life planning, facility placement and other questions you may have. Our distinguished Medical Director, Dr. Elizabethe Carter, manages symptoms such as pain, shortness of breath, nausea and anxiety and is available to our nursing staff 24/7 to be sure all needs are met for our clients and their families. When you trust your family with us, you're part of ours. As the seasons change, many of us begin preparing our homes for what is ahead. We make space. We gather what brings comfort. We notice what needs tending.

At Concho Hearts Hospice, we believe care can be an act of preparation, too— for living each day in a way that feels more supported, more comfortable, and more connected to what matters most.

That may look like managing symptoms so someone can rest more easily. It may mean helping a family find their footing, creating space for important conversations, or simply being present when the road feels uncertain.

Hospice care is not about making life smaller. It is about helping make room for peace, purpose, and the people we love.

That is the heart of Concho Hearts Hospice: compassionate care, meaningful connection, and support for the whole family through every season. Our individualized, interdisciplinary approach is designed to address physical, emotional, social, and spiritual needs alongside family support. Give us a call and discover what Concho Hearts could mean for your family this holiday season.

The season is beginning to shift.

The evenings will arrive a little earlier. The air will carry a different feeling. And before long, we will reach for familiar comforts—a favorite sweater, a warm drink, a place at the table with people we love.

At Concho Hearts Hospice, we believe the meaningful things are often the simple things: a story retold, a hand held, a song that brings someone back to a treasured moment, the comfort of being known without having to explain.

Our team walks alongside patients and families through every changing season with care that honors the whole person, and their loved ones too. We make room for comfort, connection, dignity, and the moments that matter most.

Today, we pause to remember the lives lost on September 11, 2001. We honor the courage of the first responders, service members, and everyday heroes, and hold close all those whose lives were forever changed.

At Concho Hearts Hospice, we recognize that remembrance is deeply personal. May we come together in reflection, compassion, and hope as we honor the enduring strength of our communities.

Turns out, a team that works hard also plays hard. We spent a few few days in Dallas on an action packed trip. We spent time tackling go-karts, escape rooms, and Game Show Battles (plus a little live baseball and jousting tournaments for good measure). What a wonderful trip and we came back both exhausted and energized from our time together. Feeling lucky to work, and play, with such a great crew. There was questionable driving on the go carts and lots of friendly competition in the Game Show Battle Room and the Escape Room :-) . So much fun was had with our work family!

Families trust us with so much more than their loved one’s last months.

They trust us with the details.

Medication reconciliation. Symptom management. Coordinating with physicians. Teaching families what to watch for and what deserves a midnight phone call. We are committed to a meaningful partnership with you and your loved ones, one that recognizes your individual needs and provides solutions that make sense for you.

At Concho Hearts, this means that every plan of care is built by an interdisciplinary team of nurses, physicians, social workers, chaplains, and aides, each discipline with both a unique role and a shared, active accountability. Families in our care have a clear understanding of who is doing what, and why. They know their questions and concerns will be met with compassion and professionalism.

This structure is how we honor you; curating a culture of trust. Have you been considering hospice but feel unsure of where to start? We can help. Chat with us today to learn what makes us so different.

Grief doesn’t follow a schedule.

Sometimes, it shows up on a random Tuesday, three months later, in a grocery store. It’s quiet for a while and then it isn’t. The grieving process is different for everyone, and there is no wrong way to move through it. In the hard moments, it’s okay to ask for help.

Concho Hearts offers bereavement support for families after their loved one passes. If you’re in that season and you need someone to talk to, we’re still here.
