We specialize in PERSONAL CARE in the environment where the person is: home, retirement or assisted living center, hospital, skilled nursing facility, or while traveling. Aging Life Care Management is: Assessing and Advising on Older Adult Care. Our nurse Aging Life Care Managers (ALCM) help elder adults and their families who may need guidance with long term care decisions and arrangements. AL

CM's conduct care planning assessments to identify problems and determine eligibllity for assistance and needs for specific services, like: non-medical home care, on-going medical/nursing in home monitoring, follow-up with doctors and family, etc.
